摘要
报告1例增殖型坏疽性脓皮病。患者男,63岁。双下肢反复发作疼痛性溃疡3年。皮损组织病理检查:表皮增生,细胞间水肿,真皮浅中层大量炎性细胞弥漫性浸润,以中性粒细胞为主,并可见大量核尘,真皮内部分血管壁呈纤维蛋白样坏死。过碘酸-雪夫(PAS)染色(-);真菌、细菌培养均阴性。诊断:增殖型坏疽性脓皮病;给予小剂量糖皮质激素及免疫抑制剂治疗后皮损消退,随访至今无复发。
A case of proliferative gangrenous pyoderma is reported.A 63 years old male presented with painful recurrent ulcers on his lower extremities for 3 years.Histopathology examination showed epidermal hyperplasia,intercellular edema,diffuse infiltrate of neutrophils with nuclear dusts in the upper and middle dermis,fibrin necrosis in some vascular walls in the dermis.PAS staining,fungal and bacterial cultures were negative.Diagnosis of proliferative gangrenous pyoderma was made.After given low dose of steroid and immunosuppressive agents,the skin lesions disappeared with no recurrence so far.
