早期联合应用氯吡格雷与丁苯酞注射液治疗脑梗死临床效果分析

Clinical effect of early application of clopidogrel and butylphthalide injection in treatment of patients with cerebral infarction

目的探讨急诊科早期联合应用氯吡格雷与丁苯酞注射液治疗脑梗死临床效果。方法收集94例脑梗死患者,随机分为观察组与对照组各47例,对照组采用常规治疗,观察组在对照组基础上给予早期联合应用氯吡格雷与丁苯酞注射液,观察2组治疗前后血黏度。随访6个月,观察2组患者神经功能及日常生活能力,并进行疗效对比。结果经1疗程治疗,观察组全血高切黏度(HSV)及血浆黏度(PSV)均低于治疗前及对照组治疗后(P<0.05);2组治疗前后全血低切黏度(LSV)对比,差异无统计学意义;末次随访,观察组卒中量表(NIHSS)评分较同组治疗前及对照组同期降低(P<0.05);日常生活能力量表(ADL)评分较治疗前及对照组同期提高(P<0.05);总有效率(97.87%)高于对照组(76.60%)(P<0.05)。结论早期联合应用氯吡格雷与丁苯酞注射液治疗脑梗死,可改善脑梗死预后,提高治疗效果。

Objective To explore the clinical effect of early application of clopidogrel and butylphthalide injection in treatment of patients with cerebral infarction. Methods 94 cases of cerebral infarction patients were randomly divided into observation group and control group（ n = 47）,the control group with conventional therapy,while the observation group was given early clopidogrel and butylphthalide injection on the basis of control group,before and after treatment,blood viscosity,the neurological function and daily life ability of the two groups were observed and compared during the 6 months follow-up. Results After 1 course of treatment,in the observation group,the HSV and PSV were lower than those before treatment and after treatment in the control group（ P〈0. 05）. Before and after treatment,LSV had no significant difference in the two groups; NIHSS score and ADL decreased than before treatment（ P〈0. 05）. The total efficiency（ 97. 87%） in the observation group was higher than that of the control group（ 76. 60%）（ P〈0. 05）. Conclusion Early application of clopidogrel and butylphthalide injection in treatment of cerebral infarction can improve the prognosis of cerebral infarction,improve the therapeutic effect.
