应用Beers标准对一例冠心病合并多种疾病老年患者的药学监护

Pharmaceutical monitoring care on an elderly patient with coronary heart disease combined with a variety of diseases with Beers criteria

目的探讨应用Beers标准对老年冠心病合并多种疾病患者的药学监护。方法临床药师以1例老年冠心病合并多种疾病的患者为切入点,应用Beers标准,参与治疗方案的制定,实施药学监护。结果临床药师通过优化药物治疗方案,实施药学监护,避免了潜在的药物治疗风险。结论应用Beers标准的药学监护提高了临床药物治疗水平,减少了药物不良事件发生率,保障了药物在临床治疗过程中的安全性、有效性。

Objective To explore application of Beers criteria in pharmaceutical monitoring care on elderly patients with coronary heart disease with a variety of diseases. Methods One case of elderly patient with coronary heart disease and other diseases was used as a breakthrough point,the clinical pharmacists applied the Beers criteria to the treatment plan for implementation of pharmaceutical monitoring care. Results By optimizing drug treatment,the clinical pharmacists carried out the implementation of pharmaceutical monitoring care to avoid the potential risk of drug treatment. Conclusion The application of Beers criteria in pharmaceutical monitoring care can improve the level of clinical drug treatment,reduce the incidence of adverse drug events and ensure the safety and efficacy of drugs in the clinical treatment process.
