摘要
目的初步探讨小乳腺癌的超声诊断特征及其与ER、PR、c-erBb-2表达相关性。方法选取2009年1月-2014年12月天津市第五中心医院直径小于等于2cm病理诊断为乳腺癌病例120例,回顾性观察其超声图像特征,分析其与乳腺癌的类型及ER、PR、c-erBb-2表达相关性。结果小乳腺癌超声诊断特征出现率:无包膜为100%,边缘毛刺征占43.3%,肿物回声衰减率占37.5%,纵横比≥1占36.7%,微钙化出现率占40%,血流情况Ⅰ-Ⅱ级占70.8%、Ⅲ-Ⅳ级占29.2%。有无毛刺征、不同纵横比及有无微钙化组间超声诊断准确率存在显著性差异(P<0.05);有无回声衰减及不同血流情况组间超声诊断准确率无显著性差异(P>0.05)。微钙化与c-erBb-2表达存在显著正相关(P<0.05);边缘有无毛刺征、不同纵横比、有无回声衰减及不同血流情况组间ER、PR及c-erBb-2表达均无显著性差异(P>0.05)。结论超声征象中包膜、边缘毛刺征、纵横比及微钙化有助于提高小乳腺癌的超声诊断率;微钙化的出现有助于预测c-erBb-2的表达情况,进而指导治疗及评估预后。
Objective To investigate the characteristics of ultrasound diagnosis of small breast cancer and its correlation with ER,PR and c-erBb-2.Methods 120 cases of breast cancer with diameter less than or equal to 2cm were selected from December 2014 to January 2009,and the characteristics of breast cancer were retrospectively analyzed.The correlation between 2cm and ER,PR,c-erBb-2and expression were retrospectively analyzed.Results The small breast cancer ultrasound diagnosis of occurrence rate:no capsule was 100%,spiculate margin account for 43.3%,tumor echo attenuation rate accounted for 37.5% and aspect ratio is more than or equal to 1accounted for 36.7%,micro calcification rate of 40%,blood flow grade I to II 70.8%,III-IV 29.2%.Have without spiculation,different aspect ratio and without micro calcification group between ultrasonic diagnosis accurate rate had significant difference(P〈0.05);there is no echo attenuation and different blood groups among the ultrasound diagnosis accurate rate had no significant difference(P〉0.05).There were significant positive correlation between c-erBb-2expression and micro calcification(P〈0.05);there was no significant difference in c-erBb-2,PR and ER between the edge of the edge,different aspect ratio,there is no echo attenuation and blood flow.Conclusion Ultrasonic signs capsule,spiculate margin,aspect ratio of micro calcification have helped to increase the rate of ultrasound diagnosis of small breast cancer;micro calcification appeared useful in predicting c-erbB-2expression,and guide the treatment and assessment of prognosis.
