高频彩超检查在浆细胞性乳腺炎诊断中的应用价值

目的探讨浆细胞性乳腺炎的超声诊断价值，提高诊断准确率。方法回顾性分析27例经病理证实的浆细胞性乳腺炎，总结其声像图特征。结果27例浆细胞性乳腺炎的超声诊断与术后病理诊断符合率55％（15／27），误诊为急性乳腺炎15％（4／27）、乳腺结核11％（3／27）、乳腺癌7％（2／27），未作出明确诊断11％（3／27）。其声像图表现为：肿块型15例、囊肿型6例、单纯导管扩张型4例、混合型2例。结论浆细胞性乳腺炎属增生性良性病变，因其缺乏特异性的声像图特征，诊断难度较大，高频彩超检查对此病的诊断有一定的实用价值，但需与急性乳腺炎、乳腺结核、乳癌、乳腺囊性增生等疾病鉴别。

Objective To explore the diagnostic value of ultrasound and improve the accuracy rate. Methods 27 cases of patients proved plasma cell mastitis pathology were analysed retrospectively, the features of ultrasonogram were summarised. Results The diagnostic accordance rate of ultrasound and pathology of 27 cases of plasma cell mastitis was 55% （ 15/27 ） , 4 cases were misdiagnosed acute mastitis （ 15% ） , 3 cases were misdiagnosed breast tuberculosis （ 11% ） , 2 cases were misdiagnosed breast cancer （ 7% ） , 3 cases were underdiagnosed （ 11% ） . Features of ultrasonogram were mass type （ 15 cases ） , cystic type （ 6 cases ） 、 simple duct dilatation type （ 4 cases ） , mixed type （ 2 cases ） . Conclusion Plasma cell mastitis is benign hyperplastic lesion, the diagnosis is difficult for no specific ultrasonogram.High frequency color Doppler ultrasound is of some diagnostic value, while acute mastitis, breast abscess, breast cancer, breast cystic hyperplasia need to be identified.
