异丙酚心血管麻醉对心血管应激和功能变化的影响 被引量:2

摘要 目的探讨异丙酚心血管麻醉对心血管应激和功能变化的影响。方法120例ASAⅠ~Ⅱ级全身麻醉择期手术患者,用异丙酚诱导心血管麻醉,根据异丙酚使用剂量将患者随机分为0.5 mg/kg组、1 mg/kg组和2 mg/kg组,每组40例。采用惠普监测仪监测脑电双频指数(BIS)和心率(HR);采用A-line监测仪监测听觉诱发电位指数(AAI)和麻醉诱导所需时间,比较并分析不同剂量异丙酚心血管麻醉对心血管应激和功能变化的影响。结果三组患者插管前即刻BIS和HR值均降低,与基础值间比较差异具有统计学意义(P〈0.05);插管后1 min,三组患者的BIS值低于基础值,HR高于基础值,差异有统计学意义(P〈0.05);气管插管前即刻(T1)、气管插管后即刻(T2)、气管插管后5 min(T3)时,2 mg/kg组患者的AAI值均低于0.5 mg/kg组和1 mg/kg组,差异有统计学意义(P〈0.05)。结论与其他剂量相比,2.0 mg/kg异丙酚心血管麻醉可明显改善心血管应激反应,可更好地恢复意识,对心血管损伤较小,值得临床推广应用。
