医护合作策略在主动脉内球囊反搏术患者情绪管理中的应用

Physician-nurse cooperative behavior of emotion management in patients with IABP

目的探索一种有效的患者情绪管理方法，从而提高主动脉内球囊反搏术（IABP）治疗的依从性。方法选择2013年1月—之014年7月行IABP治疗的患者，采用医护合作策略程序化管理方法：医护共同参与培训、共同决策情绪管理、制订程序化管理流程、查找隐患等，对IABP患者的情绪进行管理，比较应用该策略前后的效果。结果应用医护合作策略的程序化管理方案后，医生、护士合作状况平均得分明显高于实施前[实施前医生（19．80±3．68）分，护士（19．20±3．79）分；实施后医生（25．83±1．85）分，护士（25．78±2．15）分，差异有统计学意义（P〈0．05）]，IABP治疗期间相关不良事件发生率低于实施前：导管移位发生率由4．48％降至1．06％，过度镇静发生率由5．76％降至3．04％，IABP导管非计划性拔管发生率由0．03％降至0，差异有统计学意义（P〈0．05）。结论医护合作策略在IABP辅助治疗患者的情绪管理中具有一定优势，值得临床推广。

Objective To explore an effective emotion management method for patients treated with intra-aortie balloon pump （IABP）, in order to improve treatment compliance. Methods We recruited the patients who performed IABP from January 2013 to July 2014. All patients were conducted the physician-nurse cooperative behavior of emotion management （physieiml-nurse co-participation mode, codetermination of emotion management, establishment of procedure management flow, detection of hidden danger ere）. We Compared and evaluated the results of pre and post intervention of emotion management s^ategies in IABP patients. Results After implementing physician-nurse cooperative methods, the scores E before the implementation, doctors （ 19. 80 ±3.68） points, nurse （ 19.20 ± 3.79 ） points, after the implementation of doctors （25.83 ± 1.85 ） points, nurse （25.78 ± 2. 15） points] of physician-nurse collaboration increased signifieantly（P 〈 O. 05） ;the incidence of adverse events related to IABP treatment was below before implementation： catheter migration rate dropped from 4. 48% to 1. 06% ; the occurrence of excessive sedation rate decreased 5.76% to 3.04% ; IABP catheter unplanned extubation rate declined from O. 03% to 0 （ P 〈 0.05 ）. Conclusions The application of physician- nurse cooperative behavior of emotion management can effectively improve the therapy of IABP. That plays an important role in IABP treatment, deserves the clinical expansion.
