大剂量氨溴索对慢性阻塞性肺疾病急性加重期患者的疗效观察 被引量:5

摘要 目的:研究大剂量盐酸氨溴索对慢性阻塞性肺疾病急性加重期患者的临床疗效。方法:本研究采用随机、对照的方法,将50例需要机械通气的慢性阻塞性肺疾病急性加重期患者分为两组,在常规治疗的同时,A组加用大剂量盐酸氨溴索,B组加用常规剂量盐酸氨溴索。比较治疗第7天时两组患者pH值、动脉血氧分压(PaO_2)、二氧化碳分压(PCO_2)、动脉血氧饱和度(SaO_2)、氧合指数(PaO_2/FiO_2)、成功撤除呼吸机的患者例数及住院ICU的天数差异。结果:治疗第7天时,A组患者PaO_2、SaO_2及PaO_2/FiO_2改善优于B组患者,差异有统计学意义(P<0.01,P<0.05,P<0.001);成功撤除呼吸机的患者较B组多,差异有统计学意义(P<0.05)。A组平均住院ICU的时间比B组短,差异有统计学意义(P<0.05)。结论:大剂量盐酸氨溴索治疗慢性阻塞性肺疾病急性加重期患者,可显著改善血气参数及氧合指数、缩短机械通气及住院ICU时间,值得推广应用。 Objective:To evaluate the effect of large dosage ambroxol for acute exacerbation of chronic obstructive pulmonary disease.Methods:Fifty patients with acute exacerbation of chronic obstructive pulmonary disease which needed mechanical ventilation were divided into 2teams according to the randomized,controlled method.large dosage ambroxol was used to group A while routine dosage ambroxol used in the group B.Observe the changes of pH,PaO2,PCO2,SaO2 in blood gas analysis and oxygenation index after seven days.Results:Seven days after the treatment,PaO2,SaO2 and oxygenation index of the group A were superior to the group B,the difference had statistical significance(P〈0.01,P〈0.05,P〈0.001).The patients successfully weaning from ventilator in group A were more than the group B,the difference had statistical significance(P〈0.05).Custodial time in ICU of group A was shorter than group B,the difference had statistical significance(P〈0.05).Conclusion:Large dosage ambroxol used in patients with acute exacerbation of chronic obstructive pulmonary disease can improve the indexes of blood gas analysis and oxygenation index and shorten the time of mechanical ventilation and custodial in ICU,which is worth popularizing and applying.
