优质护理模式在妊娠期糖尿病合并妊娠期高血压产妇中的应用

Application of high quality care in gestational diabetes puerperae with pregnancy-induced hypertension

目的探讨优质护理模式对减少妊娠期糖尿病合并妊娠期高血压产妇的不良事件的效果。方法选取本院确诊为妊娠期糖尿病合并妊娠期高血压的孕产妇150例,随机均分为2组,观察组实施优质护理干预模式,对照组实施常规护理干预。观察2组的病理妊娠和并发症情况、血压等指标的改善情况、心血管事件的发生率以及患者依从性。结果观察组羊水过多、过少、早产、产后感染等病理妊娠的发生率显著低于对照组(P<0.05);干预后观察组巨大儿、死胎、胎儿窘迫、畸形儿等并发症发生率显著低于对照组(P<0.05);2组收缩压均降较治疗前降低,观察组的血压控制效果优于对照组(P<0.05);干预后观察组依从性显著优于干预前及对照组(P<0.05),对照组干预前后依从性无统计学差异(P>0.05)。结论对妊娠期糖尿病及妊娠期高血压孕产妇实施优质护理模式干预,可以明显降低病理妊娠和并发症的发生率,提高依从性,效果优于常规护理干预。

Objective To investigate high quality care in reducing adverse events of gestational diabetes puerperae with pregnancy- induced hypertension. Methods A total of 150 cases diagnosed with gestational diabetes mellitus and hypertension were randomly divided into two groups,the observation group implemented quality care intervention and control group was given routine care intervention. Pregnancy and complication situation,blood pressure improvement,the incidence of cardiovascular events and compliance of patients were observed. Results The pathological pregnancy such as polyhydramnios,oligoamnios,premature delivery,postpartum infection was significantly lower than that in the control group（ P〈0. 05）. After the intervention,complications rates in fetal macrosomia,stillbirth,fetal distress,deformed children in the observation group were lower than that in the control group（ P〈0. 05）. Systolic blood pressure in two groups were lower than treatment before,and effect of blood pressure control in the observation group was better than that in the control group（ P〈0. 05）. After intervention,adherence of patients in the observation group was significantly better than the intervention before and the control group（ P〈0. 05）. There was no significant difference in pre-intervention and post-intervention in the control group（ P〈0. 05）. Conclusion Quality care intervention for gestational diabetes and pregnancy-induced hypertension patients can significantly reduce the incidence of pathological pregnancy and complications,and improve compliance,the efficacy is superior to conventional nursing intervention.
