连续性护理干预对妊娠糖尿病患者母婴结局的影响

Effect of continuous nursing intervention on pregnancy outcomes in patients with gestational diabetes

目的分析连续性护理干预用于妊娠糖尿病患者中的效果及对母婴结局的影响。方法将本院产科接受治疗的74例妊娠糖尿病患者作为研究对象,随机分为对照组37例进行普通护理,干预组37例进行连续性护理干预。对比2组患者分娩时孕周、产后6周的糖耐量异常、2型糖尿病情况、新生儿出生体质量以及母婴结局。结果干预组患者分娩时孕周与对照组相比显著较大,而新生儿出生体质量、产后6周的糖耐量异常、2型糖尿病发病率与对照组相比显著降低(P<0.05)。干预组产妇剖宫产、早产、产后出血、羊水过多及妊娠期高血压发生率与对照组相比显著降低,干预组新生儿中巨大儿、低血糖、低体重儿及宫内窘迫发生率与对照组相比显著降低(P<0.05)。结论连续性护理干预不仅能控制患者血糖、增强患者的遵医行为,还能改善患者的母婴结局,提高其生活质量。

Objective To analyze the effect of continuous nursing intervention on maternal- infant outcome in patients with gestational diabetes. Methods A total of 74 patients with gestational diabetes treated in obstetrics department in our hospital were selected as research objects and were divided into control group with 37 cases receiving conventional nursing and intervention group with 37 cases giving continuous nursing intervention. Gestational weeks at delivery,the abnormity of glucose tolerance at 6 postpartum weeks,condition of type 2 diabetes,birth weight and pregnancy outcomes in two groups were compared. Results The gestational weeks at delivery was obviously longer,birth weight,abnormity of glucose tolerance at 6 postpartum weeks and condition of type 2 diabetes in the intervention group were obviously lower compared with the control group（ P〈0. 05）. The incidences of cesarean delivery,premature delivery,postpartum hemorrhage,polyhydramnios and gestational hypertension in the intervention group were significantly decreased compared with the control group,the incidences of macrosomia,hypoglycemia,low birth weight and intrauterine distress were significantly lower than that in the control group（ P〈0. 05）. Conclusion Continuous nursing intervention can not only control blood glucose levels,enhance treatment compliance of patients,but also improve maternal- infant outcomes and patient＇s quality of life.
