摘要
目的检测B7-H1在胰腺癌中的表达情况,初步探讨其与胰腺癌临床病理因素及预后的关系。方法应用免疫组化法和Western-blotting检测46例胰腺癌组织和5例正常胰腺组织中B7-H1的表达情况,统计学分析B7-H1和胰腺癌临床病理学特征和预后之间的关系。结果免疫组化结果发现46例胰腺癌标本中,B7-H1阳性率为56.5%(26/46),而正常胰腺组织中,未发现B7-H1阳性显色。Western-blotting结果显示在胰腺癌组织中B7-H1蛋白呈高表达(0.512±0.125);正常胰腺组织仅微弱表达(0.122±0.027),P<0.05。结合分析患者临床资料发现,B7-H1的表达与淋巴结转移、TNM分期和病理分化程度相关(P<0.05),与患者年龄、性别、肿瘤部位和大小无关。Spearman相关性分析提示肿瘤组织中B7-H1表达与血清CA19-9表达有正相关性(r=0.63)。生存曲线结果显示B7-H1阴性组胰腺癌患者较阳性组生存期明显延长(P<0.05)。Cox回归多因素分析显示B7-H1的表达是影响胰腺癌患者预后的独立因素。结论人胰腺癌组织中存在B7-Hl高表达并与胰腺癌患者的预后相关,可能是胰腺癌病情评估和预后监测的独立指标。
Objective To investigate the relationship between expression of B7-H1 and clinicopathologic characteristics and prognosis of pancreatic carcinoma. Methods The expression of B7-H1 in 46 cases of pancreatic cancer and 5 cases of normal pancreas tissues were detected by western blot and immunohistochemistry,respectively. The Spearman correlation test was used to analyze the correlation between B7-H1 expression and CA199. Survival curve was drawn by the Kaplan-Meier method,and variation of survival rate was determined by Log-rank test. Multivariate Cox proportional hazard regression model was used to analyze the independent risk factor of pancreatic carcinoma. Results The immunohistochemistry findings revealed that B7-H1 was highly expressed in carcinoma tissue(26 /46),while there were no significant expressions in normal pancreatic tissues. The Western-blotting results showed that pancreatic carcinoma samples demonstrated the up-regulated expression of B7-H1( 0. 512 ± 0. 125) at protein level compared with normal pancreas tissues(0. 122 ± 0. 027). Statistical analysis showed that the expressions of B7-H1 have relationship with lymph node metastasis,clinical stages and pathological differentiation( P〈0. 05). Spearman correlation coefficient showed association between B7-H1 expression and CA19-9 levels. B7-H1-negative expression prolonged postoperative survival when compared with corresponding positive ones( P〈0. 05) as in survival study. B7-H1-positive expression was the independent factors of prognosis assessment of pancreatic carcinoma. Conclusion These findings demonstrated the up-regulated B7-H1 expression in human pancreatic carcinoma tissues may serve as a novel biomarker for tumor progression and prognosis.
