B超引导下经皮肾镜取石术治疗复杂性肾结石的应用效果

Application Effect of Percutaneous Nephrolithotomy Surgery Guided by B Ultrasound in treatment of Complicated Renal Calculi

目的观察B超引导下经"皮肾镜取石术治疗复杂性肾结石的临床效果。方法回顾性分析山东铝业公司医院自2014年1月至2015年3月收治的复杂性肾结石患者88例,采用B超引导下经皮肾镜取石术治疗的44例患者为经皮肾镜组,采用传统开放手术进行治疗的44例患者为开放手术组。结果经皮肾镜组患者在手术时间、住院时间、下床活动时间均短于开放手术组,术中出血量少于开放手术组,两组比较差异均具有统计学意义(P<0.05);经皮肾镜组患者结石的清除率明显高于开放手术组患者,比较差异有统计学意义(93.2%vs.72.7%,P<0.05);经皮肾镜组患者并发症的发生率明显低于开放手术组患者,比较差异有统计学意义(6.8%vs.27.3%,P<0.05)。结论经皮肾镜取石术治疗复杂性肾结石与传统开放手术比较效果更好,可有效提高结石清除率,缩短手术时间和住院时间并且患者手术过程中失血量少,术后得以更快恢复,并发症相对较少。

Objective To observe the clinical effect of percutaneous nephrolithotomy surgery of complicated renal calculi under B ultrasound guidance. Methods 88 cases of patients with complicated renal calculi in our hospital from January 2014 to March 2014 were retrospectively analyzed. 44 cases of patients in the percutaneous nephroscope group were treated with B ultrasound - guided per- cutaneous nephrolithotomy and the other 44 cases of patients treated with traditional open surgery belonged to the open surgery group. Results Surgery time, the length of hospital stay, ambulation time of the patients in the pereutaneous nephroscope group were shorter than those in the open surgery group and the blood loss was also less than that in the open surgery group. The difference was statistical- ly significant （ P 〈 0. 05 ）. The stone clearance rate of patients in the percutaneous nephroseope group was significantly higher than that in the open surgery group, with statistically significant difference （93.2% vs. 72. 7% , P 〈 0.05 ）. The incidence of complications for the patients in the percutaneous nephroscope group was significantly lower than that in the open surgery group. The difference was statistically significant （6. 8% vs. 27. 3%, P 〈 0. 05）. Conclusion Percutaneous nephrolithotomy treatment for complex renal cal- culi is better than the traditional open surgery, with the advantages of increased clearance rate, shortened operation time and hospitali- zation time, less blood loss during the surgery, faster postoperative recovery and relatively fewer complications.
