摘要 目的 探讨β5t、p63和CD20在胸腺瘤中的表达及其意义。方法 观察34例胸腺瘤手术切除标本的组织学形态,采用免疫组化EnVision法检测β5t、p63和CD20在胸腺瘤中的表达,并复习相关文献。结果 (1)β5t在A型胸腺瘤中上皮细胞呈胞质并核旁点状加强阳性(4/4);在B型胸腺瘤中呈核质阳性(20/20),其中在B1型强阳性(10/10),B2型中等阳性(4/6)、弱阳性(2/6),B3中等阳性(1/2)、弱阳性(1/2),在B1型与B2、B3型中阳性强度呈逐渐下降趋势(P〈0.001);在AB型胸腺瘤中呈上述两种形态(9/9)。(2)p63在胸腺瘤上皮细胞中阳性率为60%~100%,在A型胸腺瘤中呈大片状分布(4/4),在B1、B2、B3型胸腺瘤中分别呈散在分布(10/10)、簇状或小片状分布(6/6)、大片状分布(2/2)。(3)CD20在A和AB型胸腺瘤上皮中呈局灶阳性(4/4、8/9),在B型胸腺瘤中呈阴性。结论 β5t、p63和CD20三者联合检测可以更好地对胸腺瘤进行分型,且β5t与p63相对应可有助于对B型胸腺瘤进行分型。 Purpose To study the expression of 155t, p63 and CD20 and their significance in thymoma. Methods The microscopic and immunohistochemical EnVision features were examined in 34 cases of thymoma, with review of related literatures. Results ( 1 ) 155t was expressed in the cytoplasm of neoplastic epithelial cells with paranuclear dotted expression in type A thymoma (4/4). 155t was expressed in the cytoplasm and nulcear of neoplastic epithelial cells of type B thymoma (20/20). Among the subtypes of thymoma, de- gree of expression of 155t was expressed was (+++) in type B1 ( 10/10), (++) in type B2 (4/6), ( + ) in type B2 (2/6), and (++) (1/2) in type B3. Degree of expression of 155t was declined from type B1 to B2, B3 (P 〈 0. 001 ). In type AB thymoma shows two pat- terns as in above. (2) Positive rate of p63 in thymoma was 60% ~ 100%. 1363 was expressed in large sheet in type A thymoma, scat- tered distubution in type B1, cluster or small sheet in type B2, and large sheet in type B3. (3) The experssion of CD20 was focal posi- tive in type A and AB thymoma (4/4, 8/9) and was negative in type B. Conclusion The combined expression of 155t, p63 and CD20 could be better to classification of thymoma. 155t and p63 may be better helpful for the classification of type B thymoma.
