胆囊大部切除术在老年急性坏疽性胆囊炎治疗中的价值观察

Efficacy of Subtotal Cholecystectomy in the Treatment of Elderly Acute Gangrenous Cholecystitis

目的研究分析胆囊大部切除术在老年急性坏疽性胆囊炎治疗中的价值。方法选择我院2014年7月~2015年7月接收的老年急性坏疽性胆囊炎患者68例作为研究对象,收集患者的临床资料,采用胆囊大部切除术对患者进行治疗,观察该手术的临床应用价值。结果手术时间41~80 min,平均手术时间（61.45±15.39）min,术中出血量15~96 ml,平均术中出血量（50.28±28.17）ml,引流量62~148 ml,平均引流量（82.39±10.17）ml,4例患者出现切口感染,经对症治疗后痊愈,所有患者均治愈出院。结论胆囊大部切除术在老年急性坏疽性胆囊炎治疗中的价值较高,可有效减少血管、肝外胆管损伤,促进患者尽快恢复。

Objective To study the analysis the subtotal cholecystectomy in the treatment of elderly acute gangrenous cholecystitis value. Methods From July 2014 to July 2015,receiving 68 cases of elderly acute gangrenous cholecystitis patients as the research object,collected the clinical data of patients,with most resection for the treatment of patients with gallbladder,observed the clinical application value of the operation. Results The operation time of 41 ~ 80 min,the average operation time（61.45±15.39）min,intraoperative blood loss 15 ~ 96 ml,the average intraoperative blood loss（50.28±28.17）ml,62 ~ 148 ml,the average flow rate（82.39±10.17）ml,infection of incision,observed in 4 patients recovered by symptomatic treatment. All patients were cured out of the hospital. Conclusion Subtotal cholecystectomy in the treatment of elderly acute gangrenous cholecystitis value is higher,which can effectively reduce the blood vessels and extrahepatic bile duct injury.
