全方位护理对行准分子激光原位角膜磨镶术患者围术期焦虑情绪的影响

Effect of comprehensive nursing care on perioperative anxiety of patients undergoing LASIK

目的:探讨全方位护理对行准分子激光原位角膜磨镶术(LASIK)患者围术期焦虑情绪的影响。方法:将273例行LASIK术治疗的屈光不正患者随机分为全方位护理组147例和对照组126例;对照组患者接受LASIK术围术期眼科常规护理;全方位护理组患者在常规护理基础上接受全方位护理干预,包括术前行为指导、术前心理指导、术中指导、术后用药指导和术后日常生活指导;分别于患者初诊时和麻醉前,利用焦虑自评量表(SAS)和中文版压力知觉量表(CPSS)对患者焦虑状态和压力感受评价,分别于初诊时和术中对患者血压和心率进行记录。结果:全方位护理组患者干预后SAS和CPSS评分均低于对照组(P<0.01);两组患者干预后收缩压、舒张压和心率均较干预前升高(P<0.05),全方位护理组患者干预后收缩压、舒张压和心率均低于对照组(P<0.05,P<0.01)。结论:全方位护理措施可有效缓解LASIK术患者围术期焦虑和紧张情绪,减少术中应激反应的发生。

Objective： To explore the effect of comprehensive nursing care on perioperative anxiety of patients undergoing LASIK. Methods： 273 patients with ametropia treated with LASIK surgery were randomly divided into the comprehensive nursing group（ n = 147） and the control group（ n = 126）. The patients in the control group received routine preoperative nursing care; the patients in the comprehensive nursing group were given comprehensive nursing intervention,including preoperative behavioral and psychological guidance,intraoperative guidance,postoperative medication guidance and postoperative guidance on daily life. SAS and CPSS were used to evaluate the patient＇ s anxiety status and stress at initial diagnosis and before anesthesia,and the blood pressure and heart rate of the patients were recorded at the same time. Results： The scores of CPSS and SAS were lower in the comprehensive nursing group than the control group after the intervention（ P〈0. 01）; the systolic and diastolic blood pressure and heart rate of the patients were increased in the two groups after the intervention（ P〈0. 05）; the systolic and diastolic blood pressure and heart rate of the patients were lower and slower in the comprehensive nursing group than the control group after the intervention（ P〈0. 05,P〈0. 01）. Conclusion： The comprehensive nursing measures can effectively relieve anxiety and tension during perioperative period in patients with LASIK surgery and can reduce the occurrence of stress reaction in operation.
