临床药师参与连续静—静脉血液滤过(CVVH)时万古霉素给药剂量方案设计 被引量：1

摘要 万古霉素是目前治疗耐甲氧西林葡萄球菌等感染的首选抗生素,但其具有潜在的耳、肾毒性,尤其对于合并肾功能不全且在ICU接受连续静—静脉血液滤过（CVVH）治疗的患者,合理使用万古霉素是临床药师在临床实践中的关注点。我国《万古霉素临床应用剂量专家共识（2012版）》（下称专家共识）[1]推荐对于接受置换液流速为1.5L/h的血液透析的重症感染患者,负荷剂量为25~30mg/kg（按实际体质量算）.
