中医定向透药联合局部推拿护理对关节置换术后下肢深静脉血栓的预防作用评估

Prevention Effect Evaluation of Traditional Chinese Medicine Directional Penetration Combined Local Massage Care for Lower Extremity Deep Vein Thrombosis after Joint Replacement

目的:探讨中医定向透药联合局部推拿护理对关节置换术后下肢深静脉血栓的预防作用。方法:选取243例关节置换术患者,采用回顾性分析研究方法,其中采用术后常规护理的132例为对照组,在术后常规基础上予中药定向透药联合局部推拿护理的111例为治疗组。观察两组护理前后血浆凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、D-二聚体及C-反应蛋白(CRP)。结果:治疗组护理后PT、APTT均高于对照组(P<0.05),而D-二聚体、CRP均低于对照组(P<0.05)。对照组血栓发生率为6.1%,治疗组为1.0%,差异有统计学意义(P<0.05)。结论:关节置换术后,采用中医定向透药联合局部推拿护理能降低血栓发生率,其机制可能与调节PT、APTT、D-二聚体及CRP有关。

Objective： To study prevention effect of Traditional Chinese Medicine directional penetration combined local massage care for lower extremity deep vein thrombosis after joint replacement. Methods： There were 243 patients with joint replacement were selected and retrospective study was applied; the 132 cases given the routine nursing were in the control group, and the 111 cases given Traditional Chinese Medicine directional penetration combined local massage care based on regular nursing were in the treatment group. The plasma prothrombin time（PT）, activated partial blood coagulation time（APTT）, D dimer and c-reactive protein（CRP） were determined in two groups. Results： After the nursing, the treatment group showed higher PT and APTT than control group（P〈0.05）, as well as lower D dimer and CRP than the control group（P〈0.05）. The incidence of blood clots was 6.1% in the control group and 6.1% in treatment group, and the difference was statistical（P 0.05）. Conclusion： Traditional Chinese Medicine directional penetration combined local massage care can reduce incidence of blood clots in patients with joint replacement; the possible mechanism might be related to the regulation of the PT, APTT, D dimer and CRP.
