通心络胶囊联合替罗非班对PCI术后急性心肌梗死患者支架内血栓形成和炎症因子的影响

Effect of Tongxinluo capsule combined with Tirofiban on stent thrombosis and inflammatory factors in the patients with acute myocardial infarction after PCI

目的探讨通心络胶囊联合替罗非班对PCI术后急性心肌梗死患者支架内血栓形成和炎症因子的影响。方法将90例急性心肌梗死患者随机分为观察组和对照组,对照组PCI术后进行常规治疗,观察组PCI术后予以通心络胶囊联合替罗非班治疗,比较2组支架内血栓形成情况和炎症因子水平。结果观察组术后冠脉造影TIMI血流以2级、3级为主,冠脉血流达TIMI 3级时间明显短于对照组(P均<0.05),治疗后血栓积分明显低于对照组(P<0.05);观察组术后未出现急性或亚急性血栓,2组出血事件发生率比较差异无统计学意义(P>0.05);观察组治疗后TNF-α、IL-6、hs-CRP水平均明显低于对照组,IL-10水平高于对照组,差异有统计学意义(P均<0.05)。结论急性心肌梗死患者PCI术后应用通心络胶囊和替罗非班可抑制架内血栓形成,减轻炎症反应,减少心血管事件,可在临床上推广。

Objective It is to explore the effect of Tongxinluo capsule combined with Tirofiban on stent thrombosis and inflammatory factors in the patients with acute myocardial infarction（ AMI） after PCI. Methods 90 patients with AMI were randomly divided into observation group and control group,which were treated with Tongxinluo capsule combined with Tirofiban and normal therapy after PCI. The stent thrombosis and the levels of inflammatory factors were compared between the two groups. Results In observation group,CAG showed that TIMI blood flow mainly were 2 degree or 3 degree after operation,and the reaching time of TIMI 3 degree was significantly shorter than that in control group（ P 0. 05）,thrombosis scores were lower than that in control group（ P 0. 05）. No acute or subacute thrombosis were found in observation group after operation,there was no significant difference in bleeding event occurrence between the two groups（ P 0. 05）. After treatment,the levels of TNF- α,IL- 6 and hs- CRP were lower while that of IL- 10 were higher in observation group than those in control group,the differences were significant（ P all 0. 05）. Conclusion Tongxinluo capsule combined with Tirofiban can inhibit stent thrombosis,relieve inflammatory factors and decrease cardiac events in the patients with acute myocardial infarction（ AMI） after PCI.
