中医药综合治疗对河南省农村地区HIV/AIDS患者生存质量的影响

Effects of Chinese Medicine Comprehensive Treatment on Quality of Life of Patients with HIV / AIDS in Rural Area of Henan Province

目的观察中医药综合治疗对人类免疫缺陷病毒感染者/艾滋病(HIV/AIDS)患者生存质量的影响。方法以2004年河南省参与中医项目地区的乡或村为抽样单位,整群随机抽样。参与中医项目的HIV/AIDS患者为中医治疗组,未参与中医项目的 HIV/AIDS患者为非中医治疗组。非中医治疗组给予高效抗逆转录病毒治疗(HAART)。中医治疗组给予益艾康胶囊,每次2.5 g,每日3次;由省级医疗机构医生组成的中医项目专家每月对中医项目HIV/AIDS患者巡诊,如患者有机会性感染则辨证施治给予中药汤剂治疗,直至患者退出中医项目,患者仍可参与HAART治疗。观察两组患者的一般特征,使用世界卫生组织生存质量艾滋病专用量表简表(WHOQOL-HIV BREF)调查患者的生存质量,并采用多元线性回归对生存质量影响因素进行分析。结果共有1112例研究对象纳入分析,中医治疗组569例,非中医治疗组543例。中医治疗组患者在生存质量各个领域的得分均高于非中医治疗组(P<0.05);HARRT时间与除精神领域外的各个领域呈负相关(P<0.01);年龄与生理领域、心理领域和独立性领域呈负相关(P<0.01);CD4^+计数高的HIV/AIDS患者生理领域得分高(P<0.05)。结论中医药综合治疗可以提高HIV/AIDS患者的生存质量,HARRT时间、年龄和CD4^+计数对HIV/AIDS患者生存质量有影响。

Objective To observe effects of Chinese medicine comprehensive treatment on quality of life of patients with human immunodeficiency virus / acquired immune deficiency syndrome（ HIV / AIDS）. Methods The researchers used cluster random sampling to sample patients from townships or villages in Henan Province participating in the Chinese medicine project in 2004. Patients with HIV / AIDS who participating in the Chinese medicine project were picked out into Chinese medicine treatment group,however who not participating in the project as non Chinese medicine treatment group. The non Chinese medicine treatment group was given highly active antiretroviral therapy（ HAART）. The Chinese medicine treatment group was given Yiaikang capsule（ 益艾康胶囊）,2. 5 g each time,three times a day. The Chinese medical project experts who composed of doctors from provincial or above medical institutions made a round of visits monthly to patients. If the patients had opportunistic infections,they would been given Chinese medical decoction according to syndrome differentiation till withdraw from the project. After that the patients also can participate in HAART. Then the researchers observed the general characteristics of patients in the two groups,using World Health Orgnization Quality of Life-HIV-BREF to investigate patient’s quality of life and using the multiple linear regression to analyze influence factors to quality of life. Results A total of 1112 cases were brought into the analysis including 569 cases in Chinese medicine treatment group and 543 cases in non Chinese medicine treatment group. In the Chinese medicine treatment group,scores in all fields of quality of life were higher than those in non Chinese medicine treatment group（ P 〈 0. 05）; HARRT time was negatively correlated to each field except from spiritual field（ P 〈 0. 01）; Age was negatively correlated to physiology field,psychology field and independence field（ P 〈 0. 01）; High CD4~＋count of patients with HIV / AIDS got high scores in physiology field（ P 〈 0. 05）.Conclusion Chinese medicine comprehensive treatment can improve quality of life of patients with HIV / AIDS.HARRT time,age and CD4~＋count can influence quality of life of patients with HIV / AIDS.
