摘要
目的探讨研究宫颈上皮内瘤变(CIN)临床分级与HPV分型感染的相关性。方法选取我院2012年1月至2015年1月收治的符合纳入标准的宫颈上皮内瘤变的患者200例,其中CINI-I级70例(A组),CINI-II级68例(B组),CINI-III级62例(C组),采用核酸分子快速导流杂交基因芯片技术对三组患者HPV进行分型,探讨宫颈上皮内瘤变和HPV分型感染的相关性。结果三组HPV感染阳性率分别为57.14%、85.29%和83.87%,B组和C组均较A组显著升高(P<0.05),三组单一感染率、双重感染率、三重感染率和多重感染率两两比较差异均无统计学意义(P>0.05);三组HPV31感染率两两比较,B组较C组显著降低(P<0.05),和A组比较,B组和C组HPV16和HPV58感染率差异均具有统计学意义(P<0.05),其它HPV分型感染率两两比较,差异均无统计学意义(P>0.05)。结论 CIN临床分级和HPV分型感染存在相关性,以HPV16和HPV58最为显著,可以将HPV分型检测作为判断CIN临床分级和宫颈癌病变程度的重要参考依据。
Objective To discuss and analyze the relationship between the clinical classification of cervical intraepithelial neoplasia(CIN) and HPV typing. Methods 200 patients who complied with the inclusion and exclusion standards were selected from our hospital from January 2012 to January 2015, 70 patients with grade CINI-I(A group), 68 patients with grade CINI-II(B group), and 62 patients with grade CINI-III(C group). HPV was genotyped with flow-through Hybridization method. Results The positive rates of HPV infection of group A, B and C were respectively 57.14%,85.29% and 83.87%;the positive rates of group B and group C were significantly higher than that of group A(P〈0.05). There were no significant differences in the rates of single infection, double infection and multiple infection of three groups in paired comparisons(P〉0.05). The infection rates of HPV31 of group B and C were significantly lower than that of group A; the infection rates of HPV16 and HPV58 of group B and group C were significantly different from that of group A HPV(P〈0.05). In paired comparisons, there were no significant differences in the infection rates of other HPV genotypes between groups(P〉0.05). Conclusion There was a positive correlation between CIN clinical grading and HPV genotypes. HPV16 and HPV58 infections were linked with CINI-II and III and could be used for the clinical grading of CIN and the evaluation of cervical cancer.
