IL-5、CRP在呼吸道感染患儿血浆中的表达水平及其临床意义

Clinical significance of serum levels of IL-5 and CRP in children with respiratory tract infection

目的探讨白细胞介素-5(IL-5)、C反应蛋白(CRP)水平在呼吸道感染患儿血浆中的表达及其临床意义。方法选取2012年7月至2014年6月成都天府新区人民医院收治的呼吸道感染患儿75例作为感染组,依据感染类型分为病毒组、支原体组、细菌组3个亚组,另选取同期体检中心的健康儿童30名作为对照组,所有受试者采用酶联免疫吸附法检测IL-5、CRP,采用ROC曲线分析二者对呼吸道细菌感染的诊断价值。结果感染组IL-5、CRP水平高于对照组,差异有统计学意义(P<0.05);在IL-5、CRP水平方面,细菌组>支原体组>病毒组,差异有统计学意义(P<0.05);ROC曲线分析结果显示,IL-5对小儿呼吸道细菌感染诊断特异度为63.28%,敏感度为72.59%,准确度为74.72%,CRP特异度为67.25%,敏感度为78.16%,准确度为81.26%,IL-5、CRP联合检测特异度为78.54%,敏感度为84.29%,准确度为93.19%。结论IL-5、CRP均可作为小儿呼吸道细菌感染的重要诊断指标,两者联合检测具有更高的特异度、敏感度、准确度。

Objective To discuss clinical significance of serum interleukin-5（IL-5） and c-reactive protein（CRP） levels in children with respiratory tract infection. Methods 75 children with respiratory tract infection were selected as infection group; on the basis of virus infection types were divided into virus group, mycoplasma group, bacteria group;30 cases of healthy children at the same time in physical examination center were selected as control group. Serum IL-5and CRP levels were determined by enzyme-linked immunosorbent method. ROC curve was used to analyze the diagnosis value for infantile respiratory tract bacterial infections. Results Serum levels of IL-5 and CRP of the infection group were higher than those of control group（P〈0.05）. In terms of IL-5 and CRP levels, bacteria group mycoplasma group virus group（P〈0.05）. ROC curve analysis showed that, IL-5 on pediatric respiratory tract bacterial infections showed a diagnosis rate of 63.28%, sensitivity of 72.59%, and the accuracy of 74.72%; the CRP showed a diagnosis rate of67.25%, sensitivity of 78.16%, and the accuracy of 81.26%;CRP combined with IL-5 had a diagnosis rate of 78.54%,sensitivity of 84.29%, and the accuracy of 93.19%. Conclusion IL-5 and CRP can be as an important diagnostic index of pediatric respiratory tract bacterial infections; both joint detection has higher diagnosis rate, sensitivity and accuracy.
