针刺盐包热敷法加主动功能锻炼等综合中医方案治疗肩周炎的临床观察

Clinical Observation of Applying Salt Bag Hot Compress,Three Step Method and Other Integrated Traditional Chinese Medicine Schemes in the Treatment of Scapulohumeral Periarthritis

目的：观察盐包热敷法、针刺加主动功能锻炼等综合中医方案治疗肩周炎的临床效果。方法：选取我院自2013年1月~2015年1月收治的100例肩周炎患者作为研究对象,按就诊顺序编号,以随机数字表法分为对照组与观察组2组,每组50例。对照组采用常规推拿方案治疗,观察组则采用盐包热敷法针刺加主动功能锻炼,比较2组患者治疗效果。结果：观察组治疗总有效率为98.0%,明显高于对照组的78.0%,两组对比差异有统计学意义（P〈0.05）;治疗后,观察组肩关节活动障碍及疼痛评分降低至（21.5±1.7）分、（6.5±1.4）分,与对照组相比,降低幅度更为明显（P〈0.05）;治疗1个疗程、末次治疗后观察组中医症候积分分别为（14.5±2.1）分、（6.7±1.2）分,降低幅度高于对照组,两组对比差异有统计学意义（P〈0.05）。结论：在肩周炎患者的临床治疗中,采用盐包热敷法针刺加主动功能锻炼综合方案,可提升临床治疗有效率,改善患者肩关节疼痛及障碍程度,降低中医症候积分,值得推广。

Objective： To observe the clinical effect of salt bag hot compress,three step method and other integrated traditional Chinese medicine schemes in the treatment of scapulohumeral periarthritis. Methods： 100 patients with scapulohumeral periarthritis who were admitted into the hospital during January 2013 to January 2015 were selected as the research objects. According to the order of treatment and the random number table method,the patients were divided into controlled group and observation group of two groups with 50 cases in each group. The controlled group were treated with conventional massage therapy,while the observation group were treated with salt bag hot compress combined with three step method. The curative effect was compared between the two groups. Results： The total effective rate of the observation group was 98% which was significantly higher than78. 0% of the controlled group（ P〈0. 05） and the difference between the two groups was statistically significant（ P〈0. 05）. After the treatment,the scores of shoulder joint activity dysfunction and pain decreased to（ 21. 5±1. 7） and（ 6. 5 ±1. 4） points.Compared with the controlled group,the decreasing range was more significant（ P〈0. 05）. After a course of treatment and final treatment,the integrals of TCM syndromes in the observation group were（ 14. 5 ± 2. 1） and（ 6. 7 ± 1. 2） points,respectively.The decreasing range was higher than that in the controlled group and the difference between the two groups was statistically significant（ P〈0. 05）. Conclusion： In the clinical treatment of patients with scapulohumeral periarthritis,to adopt salt bag hot compress combined with three-step comprehensive scheme can improve clinical efficiency. It can improve the degree of shoulder joint pain and dysfunction in patients and reduce the integral of TCM symptoms,which is worthy of promotion.
