摘要
目的探讨体重指数(BMI)和解剖部位对成年男性正常胰腺表观扩散系数(ADC)值的影响,进一步提高ADC值在胰腺疾病诊断中的价值。资料与方法将60例胰腺正常的成年男性志愿者按BMI分为正常体重组(BMI 18.5~23.9 kg/m^2)、超重组(BMI≥24.0~27.9 kg/m^2)和肥胖组(BMI≥28.0 kg/m^2),每组20例。在ADC图上分别测量每例受检者胰头、胰体、胰尾处的ADC值,并比较各组间及不同部位ADC值的差异。结果正常体重组胰头、胰体、胰尾ADC值均高于超重组和肥胖组(P<0.001)。超重组胰头、胰体、胰尾ADC值均高于肥胖组(P<0.001)。成年男性正常胰腺不同解剖部位的ADC值均与BMI呈负相关(r=-0.885、-0.925、-0.915,P<0.001)。结论成年男性正常胰腺ADC值会受BMI和解剖部位的影响,且随着BMI增加,胰腺ADC值呈下降趋势,在对胰腺疾病进行诊断时要考虑到BMI和解剖部位的影响。
Purpose To investigate the effect of body mass index(BMI) and anatomic location on apparent diffusion coefficient(ADC) value in the normal male pancreas so that the ADC value in the diagnosis of pancreatic disease could be improved. Materials and Methods Sixty male volunteers with normal pancreas were categorized into three groups according to BMI with 20 people in each group, which were normal group(BMI 18.5-23.9 kg/m^2), overweight group(BMI 24.0-27.9 kg/m^2) and obese group(BMI ≥ 28.0 kg/m^2). The ADC value was calculated with ROIs positioned in the pancreas head, body and tail regions. The differences of ADC value were compared in each group and at different regions of the pancreas. Results The ADC values of pancreas head, body and tail regions in the normal group were all higher than those in the overweight group and obesity group(P〈0.001). In the overweight group, the ADC values of pancreas head, body and tail regions were higher than those in the obesity group(P〈0.001). BMI was negatively correlated with the ADC value of pancreas head, body and tail regions in normal males(r=-0.885,-0.925,-0.915, P〈0.001). Conclusion BMI and anatomic location can affect the ADC value of normal male pancreas. The ADC value would decrease along with the increase of BMI. Therefore, the consideration of BMI and anatomical location is meaningful in the diagnosis of pancreatic disease.
