摘要
目的:计数慢性乙型肝炎病毒(HBV)感染者肝脏组织中CD4^+T、CD8^+T和Fox P3^+Tregs细胞,观察不同免疫状态下的肝脏组织中CD4^+T和CD8^+T细胞表达水平之间的差异,探索其临床意义。方法:选取慢性HBV感染者68例,健康对照组(非HBV感染组)30例,运用免疫组织化学的方法进行检测。结果:慢性HBV感染组的CD4^+T、CD8^+T和Fox P3^+Tregs细胞表达水平明显升高,与非HBV感染组相比,P均<0.001;在HBV感染组中,CD4^+T和CD8^+T细胞在免疫耐受期患者肝脏组织中的表达水平最低,CD4^+T、CD8^+T细胞在免疫清除期、再活动期中的表达水平与二者在免疫耐受期相比,均有增高趋势,然而CD4^+T、CD8^+T细胞的表达水平在免疫耐受期、免疫清除期、低复制期及再活动期任意两期之间比较,P均>0.05。结论:CD4^+T和CD8^+T细胞在免疫耐受期、免疫清除期、低复制期及再活动期患者肝脏组织中表达水平的变化趋势在一定程度上反应了各期肝脏的免疫状态;Fox P3^+Tregs细胞可能与HBV感染慢性化有关。
Objective: To count the number of CD4^+T, CD8^+T and Fox P3^+Tregs in the liver tissue of patients with chronic HBV infection and compare the expression levels of CD4^+T and CD8^+T cells in liver tissues in different immune status. Methods: The levels of CD4^+T,CD8^+T and Fox P3^+Tregs were detected by the method of immunohistochemistry in 68 chronic HBV infection patients, and 30 individuals without HBV infection as healthy control. Results: Compared with the non-HBV infection group, the expression level of CD4^+T, CD8^+T and Fox P3^+Tregs were significantly higher in the chronic HBV infection group(P〈0.001); In chronic HBV infection group, the numbers of CD4^+T and CD8^+T in immune clearance and active period were higher than the ones in the immune tolerance, and the liver tissue from immune tolerance patients had lowest CD4^+T and CD8^+T levels, but P〈0.05 respectively. Conclusion: To some extent, the expression levels of CD4^+T and CD8^+T cells in the immune tolerance, immune clearance phase, low copy phase and active period indicate the liver immune status; and maybe Fox P3^+Tregs are associated with chronic HBV infection.
