飞秒激光辅助Q值调整的非球面个性化LASIK与常规LASIK术后视觉质量对比研究 被引量:6

摘要 目的通过对近视患者行Q值调整LASIK术和常规的LASIK术术后屈光度、裸眼视力、Q值、高阶像差、视觉对比敏感度的观察,对其视觉质量进行对比评估,同时也为屈光手术病人选择更好地手术方式提供理论依据。方法临床病例对照研究。2014年1-6月在石家庄爱尔眼科医院接受LASIK手术治疗近视患者,分为二个组:Q值调整的非球面个性化LASIK组、常规LASIK组,术后随访6个月观察裸眼视力、Q值、屈光度、高阶像差、对比敏感度和患者术后主观视觉质量调查等情况。结果两组病人术后屈光状态、术后裸眼视力、预测性均达到理想状态,两组差异无统计学意义(P〉0.05)。两组6个月Q值、高阶像差(球差)比P〈0.05,有统计学意义。暗环境下对比敏感度:眩光条件下,第三、四等级标准组未通过例数增加更显著,两组差异有统计学意义(P〈0.05)。主观视觉感受:Q值调整的非球面个体化LASIK组比常规LASIK组主观视觉感受度稍佳,具有统计学意义(P〈0.05)。结论Q值调整的非球面个性化LASIK组与常规的LASIK术相比,术后球差的变化小于常规组,且提高了患者术后的视觉质量,减少了夜视力下降、眩光、对比敏感度异常的发生。 Objective To assess and compare the postoperative visual quality of the Q value ad- justed LASIK and conventional LASIK surgery through uncorrected visual acuity, Q value, higher-or- der aberrations, visual contrast sensitivity for evaluation, and to provide a theoretical basis for myo- pic patients choose better refractive surgery. Methods Myopia patients from January 2014 to June in Shijiazhuang ophthalmic hospital were divided into two groups: Q value adjustment of corneal re- fractive surgery group, the conventional LASIK group received treatment of LASIK surgery, and were followed-up 6 months postoperatively. The naked eye sight, Q value, diopter, higher-order aber- rations, contrast sensitivity, and patients with postoperative subjective visual quality investigation were observed. Results Postoperative refractive status and naked eye eyesight achieved predictive ideal state on both groups, and there were no statistical differences in the two groups, while there was statistically significant P 〈0.05 on two groups of six months Q value, high order aberration (spherical aberration). Dark environment contrast sensitivity, glare, under the condition of the third and fourth grade standard group failed cases was increased significantly, the difference between the two groups was statistically significant (P 〈0.05). Subjective visual perception: Q value adjustment of aspheric excimer laser group than conventional LASIK there was a slightly better subjective visual perception degrees, with statistical significance (P 〈0.05). Conclusions Q value adjustment of cor- neal refractive surgery compared with conventional LASIK surgery, postoperative spherical aberration changes less than routine group, and improve the quality of the patients postoperative visual, reduce the night vision loss, glare, the occurrence of abnormal contrast sensitivity.
