益气养阴活血汤联合贝那普利治疗气阴两虚兼血瘀型早期糖尿病肾病随机平行对照研究

Effect of Yiqi Yangyin Huoxue Decoction combined with Benazepril in the Treatment of Deficiency of both Qi and Yin and Blood Stasis Type of Early Diabetic Nephropathy Randomized Parallel Controlled Study

[目的]观察益气养阴活血汤联合贝那普利治疗气阴两虚兼血瘀型早期糖尿病肾病疗效。[方法]使用随机平行对照方法,将60例门诊患者按就诊顺序编号方法简单随机分为两组。对照组30例贝那普利,10~20mg/d,1次/d,口服。治疗组30例益气养阴活血汤（黄芪30g,太子参、生地各15g,麦冬12g,山茱萸、白术各10g,茯苓20g,川芎、当归、丹参各10g,大黄~制6g）1剂/d,水煎400mL,早晚分服;西药治疗同对照组。连续治疗2个月为1疗程。观测临床症状、空腹血糖、餐后血糖、糖化血红蛋白、血尿素氮、肌酐、尿白蛋白排泄率、不良反应。治疗1疗程,判定疗效。[结果]治疗组显效18例,有效11例,无效1例,总有效率96.68%。对照组显效10例,有效14例,无效6例,总有效率80.00%。治疗组疗效优于对照组（P〈0.05）。空腹血糖、餐后血糖、糖化血红蛋白两组均有改善（P〈0.01,P〈0.05）,治疗组改善优于对照组（P〈0.01,P〈0.05）;血尿素氮、肌酐、尿白蛋白排泄率两组均有改善（P〈0.01,P〈0.05）,治疗组改善优于对照组（P〈0.05,P〈0.01）。[结论]益气养阴活血汤联合贝那普利治疗气阴两虚兼血瘀型早期糖尿病肾病,疗效满意,无严重不良反应,值得推广。

[Objective]To observe the Yiqi Yangyin Huoxue Decoction Combined with benazepril in treatment of deficiency of both Qi and Yin and blood stasis type early diabetic nephropathy.[Methods]60patients were randomly divided into two groups according to the order of treatment.The control group of 30 cases of benazepril,10~20mg/d,l times /d,oral.Treatment group（30 cases） was treated with Yiqi Yangyin Huoxue Decoction（Radix Astragali 30 g,Radix Pseudostellariae,radix rehmanniae each 15 g,12g of the tuber of dwarf lilyturf,dogwood,atractylodes each 10 g,Poria cocos 20 g,Ligusticum chuanxiong and Angelica sinensis,Radix Salvia miltiorrhiza each 10 g,prepared rhubarb 6g） agent 1 / d,water to cook 400 ml.Sooner or later the service of Chinese medicine and western medicine with the control group.Continuous treatment for2 months for 1 courses.Observation of clinical symptoms,fasting blood glucose,postprandial blood glucose,glycosylated hemoglobin,urinary albumin excretion rate,renal function indicators,adverse reactions.Treatment of 1 courses,determine the efficacy.[Results]18 cases were markedly effective in the treatment group,11 cases were effective,1 cases were ineffective,the total effective rate was 96.68%.In the control group,10 cases were markedly effective,effective in 14 cases,invalid in 6 cases,the total effective rate was 80%.The curative effect of the treatment group was better than that of the control group（P〈0.05）.Two groups of fasting blood glucose,postprandial blood glucose,glycosylated hemoglobin were improved（.P〈0.01,P〈0.05）,the treatment group were better than the control group（P〈0.05）;two groups of blood urea nitrogen,creatinine,urinary albumin excretion rate were improved（P〈0.01,P〈0.05）,the treatment group were better than the control group（P〈0.05,P〈0.01）.[Conclusion]Yiqi Yangyin Huoxue decoction combined with benazepril in treatment of deficiency of both Qi and Yin and blood stasis type early diabetic nephropathy,and the results were satisfactory.No serious adverse reactions,worthy of promotion.
