针刺与TDP照射联合羌防归芎饮与功能锻炼治疗面神经炎115例临床观察

Acupuncture and TDP Irradiation combined with Anti Qiang GUI Xiong Decoction With Functional Exercise for The Treatment of Facial Paralysis Clinical Observation of 115 Cases

[目的]观察针刺与TDP照射联合羌防归芎饮与功能锻炼治疗面神经炎疗效。[方法]使用前瞻性设计方法,对115例门诊患者使用针刺联合羌防归芎饮治疗。针刺:取坐位或平卧位,主穴:阳白、太阳、地仓、四白、攒竹、双侧合谷、太冲;配穴:抬头纹消失、闭目困难配鱼腰、攒竹、丝竹空,口歪重配人中颧醪、承浆;取0.35×20mm毫针,患侧阳白、太阳、颧醪、四白、鱼腰、攒竹、丝竹空均直刺,手法宜轻,不宜捻针,人中透地仓、承浆透地仓、地仓透颊车以平补平泻手法为主,双侧合谷、双侧太冲均以泻法为主;留针30mim,行针1次/10min。TDP波普治疗仪照射患侧(双目须纱布覆盖),1次/d,10min/次。羌防归芎饮,全蝎^(冲服)6g,僵蚕10g,白附子5g,蜈蚣^(冲服)2条,白芷、钩藤、川芎、防风各10g,桂枝6g,葛根10g,羌活6g,1剂/d,水煎200mL,早晚温服。功能锻炼:皱眉、抬额、闭眼、露齿、鼓腮、吹口哨等动作训练,数分钟/次,数次/d。连续治疗7d为1疗程。观测临床症状、表情肌功能、不良反应。连续治疗3疗程,判定疗效。[结果]痊愈99例,显效11例,有效4例,无效1例,总有效率99.13%(114/115)。[结论]针刺与TDP照射联合羌防归芎饮与功能锻炼治疗面神经炎,疗效满意,无严重不良反应,值得推广。

[Objective]To observe the curative effect of acupuncture and TDP irradiation combined with anti Qiang GUI Xiong decoction with functional exercisefor the treatment of facial neuritis.[Methods]The use of forward-looking design method,115 cases of outpatients using acupuncture combined with anti Qiang GUI Xiong decoction.Take a sitting or supine,routine disinfection of the acupoints,the main point：Bai Yang,sun,warehouses,Sibai,save bamboo,bilateral Hegu and Taichong.Acupoints：wrinkles disappear,eyes closed with difficulty Yuyao,cuanzhu,sizhukong,or people with crooked mouth and Quanliao,chengjiang.Needle selection x 0.35 mm needle,suffering from side Bai Yang,sun,Quanliao,Sibai,fish waist,save bamboo,sizhukong are perpendicularly,manipulation should be light.not acupuncture.People through Dicang,bearing slurry through the barn,silo through buccal car to reinforcing reducing manipulation as the main,bilateral Hegu.bilateral LiV3 in reducing method mainly.For 30 min,each 10 min a time line,and with the TDP irradiation treatment of ipsilateral pop（binocular to gauze covering l/d）.Anti Qiang GUI Xiong drink,scorpion blunt 6g,Bombyx Batryticatus10 g,white aconite 5g,centipede Chongfu 2g,dahurian angelica root,uncaria,chuanxiong,Fangfeng each 10 g,Guizhi6g,kudzu root 10 g,notopterygium root 6g,daily 1 agent,water simmer in Water 200 ml,morning and evening clothes.Functional exercise,frowning,lift the forehead,close your eyes,teeth,drum,gills,whisding training minutes/times,several times/d.7d is a continuous treatment for 1 courses.Observation of clinical symptoms,facial expression muscle function,adverse reactions.Continuous treatment of 3 courses,determine the efficacy.[Results]99cases were cured,11 cases markedly effective,4 cases effective,1 cases ineffective,the total effective rate was99.13%（114/115）.[Conclusion]acupuncture and TDP irradiation combined with anti Qiang GUI Xiong decoction With functional exercisefor for the treatment of facial neuritis,satisfactory curative effect,no serious adverse reactions,worthy of promotion.
