糖调节受损患者瘦素与颈动脉内膜中层厚度变化的关系 被引量:1

摘要 目的:探讨糖调节受损(IGR)患者瘦素水平及其与颈动脉内膜中层厚度(IMT)变化的关系。方法:选择IGR患者118例,根据口服葡萄糖耐量试验(OGTT)结果,分为以下3组,单纯空腹血糖受损组(IFG组)38例,单纯糖耐量减低组(IGT组)42例,IFG合并IGT组(IFG+IGT)组38例,随机选择同期体检的健康人群(NGT组)42例,检测4组患者的血脂、空腹血糖(FBG)、空腹胰岛素(insulin)、空腹C肽、餐后2 h血糖(PBG)、餐后2 h胰岛素(2 h-insulin)、餐后2 h C肽(2h-C肽)、糖化血红蛋白(HbA1C)、C-反应蛋白(CRP)、体重指数(BMI)、瘦素、IMT,应用多元线性回归分析瘦素和IMT的关系,进一步应用pearson相关分析研究瘦素与IMT的关系密切程度。结果:组间比较显示,与NGT组比较,IFG组、IGT组、(IFG+IGT)组的IMT增厚,瘦素水平增高,差异有统计学意义(P<0.05);与IFG组、IGT组相比,(IFG+IGT)组的IMT增厚,瘦素水平增高差异有统计学意义(P<0.05)。多元线性回归分析显示,在(IFG+IGT)组人群中,瘦素与IMT增厚具有相关性(P<0.05),而IFG组、IGT组无相关性(P>0.05)。结论 :IGR人群中瘦素水平增高,颈动脉内膜中层厚度增加,空腹血糖受损合并糖耐量减低人群中瘦素水平与IMT关系密切。 Objective To investigate the relationship between leptin level of IGR patients and intima- media thickness (IMT) of carotid arterial. Methods One hundred and eighteen IGR patents were enrolled into the following three groups according to the result of oral glucose tolerance test (OGTI') : 38 cases of the purely impaired fasting glucose (IFG) group, 42 cases of the purely impaired glucose tolerance (IGT) group and 42 cases of the IFG combining with IGT (IFG + IGT) group. Thirty-eight cases with normol glucose tolerance (NGR) were enrolled in the NGR group. The blood fat, fasting blood-glucose (FBG), fasting insulin (insulin), fasting C peptide, 2 h-postprandial blood glucose, 2 h-insulin, 2 h-C peptide, glycosylated hemoglobin (HbA1C), C-reactive protein (CRP), body mass index (BMI), leptin and IMT were determined in patients of the three groups and the healthy controls. The multiple linear regression was used to analyze the relationship between leptin level and IMT, and person correlation was further used to analyze the affinity degree between leptin level and IMT. Results Compared with the NGT group, the inter-group comparison showed that leptin level was significantly increased in the IFG, IGT and (IFG + IGT) groups (P 〈 0.05, respectively). Compared with the IFG and IGT groups, IMT and the leptin level were increased in the (IFG + IGT) group (P 〈 0.05, respectively). The multiple linear regression analysis showed that leptin level was correlated with the increase of IMT in the (IFG + IGT) group (P 〈 0.05), but not in the IFT group and the IGT group. Conclusion The leptin level in the IGR group and the IMT in the IFG + IGT) group were significantly increased, the leptin level was closely associated with IMT.
