ProGRP和NSE对小细胞肺癌的诊断及疗效评估的价值

Diagnostic and Curative Evaluation Value of ProGRP and NSE in Small Cell Lung Carcinoma

目的:探讨胃泌素释放肽前体(Pro GRP)和神经元特异烯醇化酶(NSE)对小细胞肺癌(SCLC)的诊断及疗效评估的价值。方法:收集在2014年1月至2015年10月蚌埠医学院第一附属医院呼吸科住院首次确诊并化疗SCLC患者60例,初治非小细胞肺癌(NSCLC)患者60例,肺良性疾病(BPD)患者40例,分别采用ELISA法和放射免疫分析法检测患者血清中Pro GRP和NSE的水平,评估其对SCLC的临床价值。结果:(1)血清Pro GRP和NSE在SCLC组中的水平明显高于NSCLC组和BPD组(均P<0.05)。(2)Pro GRP和NSE在广泛期SCLC组的水平均高于局限期SCLC组(P<0.05)。(3)采用ROC曲线确定Pro GRP诊断SCLC的界值为64.68 pg/m L,Pro GRP诊断SCLC的敏感度、特异度和约登指数分别为86.7%、97.5%、0.842,明显高于NSE(P<0.05)。(4)化疗2周期后,疗效为疾病控制组SCLC患者血清Pro GRP水平较治疗前明显降低(P<0.05),而疗效为疾病进展组SCLC患者血清Pro GRP水平与治疗前相比差异无统计学意义(P>0.05)。结论:Pro GRP、NSE可以作为SCLC诊断和疗效评估的标志物,Pro GRP较NSE具有更高的灵敏性和特异性,可应用于临床。

Obejective To explore the diagnostic and curative evaluation value of gastrin-releasing peptide precursor（Pro GRP） and neuron specific enolization enzyme（NSE） in small cell lung cancer（SCLC）.Methods Sixty SCLC patients, sixty non-small cell lung cancer（NSCLC） patients and forty patients with benign pulmonary disease were collected from January 2014 to October 2015. The levels of serum Pro GRP and NSE in all patients were determined by ELISA method and radioimmunoassay respectively then the clinical value of Pro GRP and NSE on SCLC was evaluated. Results The levels of Pro GRP and NSE in SCLC group were significantly higher than those in NSCLC group and those in lung benign disease group（P 0.05）. The levels of ProGRP and NSE in extensive stage were higher than those in limited stage in SCLC group（P〈0.05）. The boundary value of SCLC through Pro GRP identified through ROC curve was 64.68 pg/mL. The diagnostic sensitivity,specific degree and Youdenindex of Pro GRP in SCLC were 86.7%, 97.5% and 0.842 respectively, which were significantly higher than NSE（P〈0.05）. After 2 cycles of chemotherapy, serum Pro GRP in SCLC disease control groupwere significantly decreased（P〈0.05） but on difference of serum Pro GRP was found in SCLC progressiongroup（P〉0.05）. Conclusion Pro GRP and NSE can be used as markers for the diagnostic and curative evaluation of SCLC. And Pro GRP has higher sensitivity and specificity than NSE and can be promoted in clinic.
