奥沙利铂联合氟尿嘧啶治疗晚期胃癌的近远期疗效观察

The Short-term and Long-term Efficacy of Oxaliplatin Combined with Fluorouracil for Advanced Gastric Cancer

目的探讨奥沙利铂联合氟尿嘧啶治疗晚期胃癌的近远期疗效、生存时间以及不良反应。方法将169例晚期胃癌患者随机分为观察组(奥沙利铂+氟尿嘧啶组)85例,对照组(顺铂+5-氟尿嘧啶组)84例,将2组的ORR、DCR、癌症复发、转移、3年的生存率以及不良反应发生率进行比较。结果 2组在治疗晚期胃癌方面均具有良好的疗效,P均>0.05,差异无统计学意义。观察组胃癌复发13例(15.29%),癌症转移9例(10.59%),3年的生存率为74.12%;对照组胃癌复发12例(14.29%),癌症转移11例(13.10%),3年的生存率为72.62%。2组胃癌复发率、癌症转移率以及3年生存率进行比较,P均>0.05,均无统计学差异。2组患者进行3周化疗后均出现不同程度的不良反应,3～4级不良反应发生率均较低,观察组的不良反应发生率低于对照组,差异具有统计学意义,P<0.05。结论奥沙利铂联合氟尿嘧啶治疗晚期胃癌的临床缓解率以及临床控制率较高,不良反应轻,值得临床应用推广。

Objective To investigate the short-term and long-term efficacy,survival time and adverse reactions of oxaliplatin combined with fluorouracil for advanced gastric cancer. Methods 169 cases of advanced gastric cancer were randomly divided into the observation group（ oxaliplatin combined with fluorouracil） and the control group （oxaliplatin combined with cisplatinure）, the observation group had 85 cases, and the control group had 84 cases. The ORR, DCR, cancer recurrence, metastasis ,3- year survival rate and adverse reactions rate of the 2 groups were compared. Results The 2 groups both had good efficacy,which had no significant difference,P 〉0.05. The observation group had 13 cases of cancer recurrence（ 15.29% ）, 9 cases of cancer metastasis （10.59%）, 3-year survival rate was 74.12%, and the control group had 12 cases of cancer recurrence （14.29%）, 11 cases of cancer metastasis（ 13.10% ） and 3-year survival rate was 72.94%. The gastric cancer metastasis ,cancer recurrence and 3-year of survival rate of the 2 groups had no significant difference,P 〉0.05. The 2 groups had different degree of adverse reactions, the 3 - 4 grade adverse reactions of the 2 groups were low, and adverse reactions of the observation group was lower than that of the observation group （P 〈 0.05 ）. Conclusion Oxaliplatin combined with fluorouracil have high clinical releasing rate and clinical controlling rate, less adverse reactions and it is worthy of clinical application.
