前牙美学区即刻种植即刻非功能修复的美学效果评价

Aesthetic effect of immediate implant placement and non-functional repair for anterior aesthetic zone

目的观察并探讨前牙美学区即刻种植即刻非功能修复的美学效果。方法选择自2011年5月至2014年10月在本医院口腔科就诊的前牙美学区牙列缺损的患者20例,共20颗上前牙,微创拔除后即刻植入20枚Xive种植体,当天完成固定修复,6个月内完成最终修复,作为观察组。选取同期行延期种植的20例患者(20枚种植体)作为对照组。随访6个月、12个月后X线片骨吸收值和牙龈乳头指数得分,运用红色美学指数(PES)评分对种植体修复后的软组织情况进行评价。并测量两组患者的主观满意度。结果所有种植体骨结合良好,均完成修复,患者恢复咬合功能;观察组6个月、12个月后X线片骨吸收值、24个月后X线片骨吸收值和牙龈乳头指数得分分别为(0.68±0.12)mm、(0.32±0.04)mm、(0.28±0.01)mm和(3.12±0.24)分,对照组为(1.28±0.10)mm、(0.30±0.06)mm、(0.25±0.02)mm和(2.52±0.34)分,观察组6个月后X线片骨吸收值明显低于对照组,而牙龈乳头指数得分明显高于对照组(P<0.05);观察组3个月和6个月PES得分均明显高于对照组(P<0.05);观察组满意率分别为95.0%(19/20),对照组满意率为75.0%(15/20),两组相比,差异具有统计学意义(P<0.05)。结论即刻种植即刻非功能修复前牙美学区牙列缺损在严格掌握适应证的基础上能获得更好的美学效果,满意度高。

Objective To observe and explore the aesthetic effect of immediate implant placement and nonfunctional repair for anterior aesthetic zone. Methods Twenty patients with anterior aesthetic zone dentition defect accepted dental implantation surgery in this hospital since May 2011 to October 2014 were selected,20 Xive implant was implanted immediately after minimally invasive,fixed prostheses at same day,completed the final repair within 6 months,as the observation group. 20 patients（20 implants）concomitant delayed implant was selected as the control group. X-ray bone resorption values was followed for 6 months,12 months and gingival papilla index scores,red aesthetic index（PES）ratings for soft tissue implants restored the situation was evaluated. And subjective satisfaction of the two groups was measured. Results The osseointegration of all implants was good,with well occlusal function recovered; X-ray bone resorption values at 6 months,12 months,24 months and papilla index scores were（0.68 ± 0.12）mm,（0.32 ± 0.04）mm,（0.28 ± 0.01）mm and（3.12 ± 0.24）points in the observation group,while was（1.28 ± 0.10）mm,（0.30 ± 0.06）mm,（0.25 ± 0.02）mm and（2.52 ± 0.34） points in the control group,X-ray bone resorption values at six months in the observation group were significantly lower than control group,while the papilla index score was significantly higher（P〈0.05）; 3 months and 6 months PES scores were significantly higher in the observation group（P〈0.05）; satisfaction rate was 95.0%（19/20） in the observation group,while was 75.0%（15/20） in the control group,the difference was statistically significant（P〈0.05）. Conclusion Immediate implant placement and non-functional restoration for anterior dentition defect in the aesthetic zone can obtain better aesthetic effect on the basis of indications were strictly controlled,satisfaction is high.
