去氧孕烯炔雌醇片与甲羟孕酮治疗青春期功能失调性子宫出血临床效果比较

Clinical efficacy of desogestrel and ethinylestradiol tablets compared with medroxyprogesterone in puberty dysfunctional uterine bleeding

目的 观察比较甲羟孕酮与去氧孕烯炔雌醇片治疗青春期功能失调性子宫出血的临床疗效。方法 选取嵊州市人民医院妇产科自2014年3月~2015年6月期间收治疗的确诊为青春期功能失调性子宫出血的患者共72例,按随机数字表法分为观察组和对照组各36例,对照组给予甲羟孕酮治疗,观察组给予去氧孕烯炔雌醇片治疗,随访并记录2组患者出血控制时间、完全止血时间、子宫内膜厚度、经量、经期、疗效情况。结果 观察组患者治疗后出血控制时间、完全止血时间分别为（10.34±2.02、21.34±4.24）d,均短于对照组（15.67±2.61、28.35±4.59）d（P〈0.05）。观察组患者治疗后子宫内膜厚度、经量、经期值分别为（0.41±0.10）cm、（50.49±6.92）m L、（4.35±0.52）d均小于对照组（0.71±0.15）cm、（89.31±8.34）m L、（7.61±0.84）d（P〈0.05）。观察组治疗后总有效率为97.22%,高于对照组80.56%（P〈0.05）。结论 去氧孕烯炔雌醇片治疗青春期功能失调性子宫出血的临床治疗效果确切,无明显副反应,效果优于甲羟孕酮。

Objective To observe the clinical efficacy of desogestrel and ethinylestradiol tablets compared with medroxyprogesterone in treatment of patients with puberty dysfunctional uterine bleeding. Methods 72 cases from March 2014 to June 2015 in our hospital were randomly divided into observation group and control group, 36 cases in the control group were given medroxyprogesterone treatment, and observation group were given desogestrel and ethinylestradiol tablets treatment. The two groups were followed up and bleeding control time, complete hemostasis time, endometrial thickness, amount of menstruation, menstruation and treatment effect were recorded. Results After treatment, bleeding control time and complete hemostasis time in observation group were （ 10.34 ± 2.02, 21.34 ± 4.24 ） d, which were shorter than control group （ 15.67 ± 2.61,28. 35 ± 4. 59 ） d（ P 〈0. 05）. The endometrial thickness, amount of menstruation and menstruation were （0.41 ±0. 10）cm, （50. 49±6.92）mL, （4. 35 ±0.52） d, which were lower than control group （ 0. 71 ± 0. 15 ） cm, （ 89. 31 ± 8. 34 ） mL, （ 7.61 ± 0. 84 ） d （ P 〈 0. 05 ）. After treatment, the total effective rate in observation group was 97.22% ,which was higher than 80. 56% in control group（P 〈 0.05 ）. Conclusion Desogestrel and ethinylestradiol tablets has exact clinical efficacy with no obviously adverse reaction in patients with puberty dysfunctional uterine bleeding, which is better than medroxyprogesterone.
