医师职业精神评价指标体系构建研究——以上海市8所三级甲等医院为例

Research on the Construction of Evaluation Index System for Physician Professionalism——Based on 8 Three-A Hospitals in Shanghai

目的为有效检验及提升医师职业精神状态,初步构建医师职业精神评价指标体系。方法抽取来自上海8家三级甲等医院的临床医师240名,采用自设问卷调研的形式,运用层次分析法构建医师职业精神评价模型并确定各指标权重。结果初步构建了医师职业精神评价指标体系,在道德品质特征、成就行动特征、职业认知特征3项一级指标下筛选出责任心等14项二级特征要素,并确定了各一级指标及二级特征要素的权重分布,经一致性检验,证明判断矩阵具有完全的一致性,说明该权重系数能够较好地反应指标的相对重要程度。结论道德品质特征是医师职业精神评价的重要构成要素;责任意识及职业理想对于医师的成长至关重要;终身学习及沟通能力的高权重体现了医疗行业发展所需;医学伦理意识在医师群体中有逐渐强化的趋势;进一步提升医师职业精神评价体系的可操作性。

Objective： To inspect and improve physician professionalism effectively and preliminarily build evaluation index system for physician professionalism. Methods： A sample of 240 physicians selected randomly from 8Three- A hospitals in Shanghai were investigated using a self- designed questionnaire. Then the method of analytic hierarchy process was performed in this study to construct the evaluation model of physician professionalism and determine every index weight. Results： An index system was initially built for physician professionalism evaluation. This system comprised three level- 1 indexes namely moral quality characteristics,achievement action characteristics and vocational cognitive characteristics,as well as 14 level- 2 indexes that drew from level- 1 including responsibility etc.,at the same time determined their weight distribution. The judgment matrix had completely consistency through consistency check,indicating that the weight coefficient can reflect the relative importance of every index. Conclusion： Moral quality characteristics is one of the most important components of physician professionalism evaluation; responsibility consciousness and vocational ideal are vital for the growth of physicians; lifelong learning and communication skills with high weight reflects needs of modern health care industry development;medical ethics consciousness has a trend of strengthening in physician groups,and enhancing evaluation index system for physician professionalism has the operability.
