低剂量雌激素替代疗法治疗围绝经期综合征及对Kupperman评分和激素水平的影响分析

The curative effect and effect on Kupperman score and hormone levels in the treatment of low dose estrogen replacement therapy on the perimenopausal syndrome

〔目的〕探讨低剂量雌激素替代疗法治疗围绝经期综合征的疗效及对Kupperman评分和激素水平的影响。〔方法〕选取围绝经期综合征患者132例,随机分为两组,各为66例。对照组每日口服孕激素甲羟孕酮,4 mg/d;研究组在对照组基础上口服雌激素戊酸雌二醇片,0.65 mg/d,两组均连续治疗3 mon。比较两组治疗前后Kupperman症状评分,激素雌二醇(E2),促卵泡素(FSH)和黄体生成素(LH)水平及子宫体积和内膜厚度变化,比较两组不良反应。〔结果〕治疗3 mon后,研究组Kupperman评分显著低于治疗前和对照组(P<0.05);研究组E2水平显著高于治疗前和对照组水平(P<0.05),LH和FSH水平显著低于治疗前和对照组水平(P<0.05);子宫体积和内膜厚度与治疗前和对照组比较无显著差异(P>0.05);两组不良反应间无显著差异(P>0.05)。〔结论〕低剂量雌激素替代疗法能促进围绝经期综合征患者体内激素分泌的正常化,显著改善临床症状,不良反应少,具有显著疗效。

〔Objective〕To investigate the curative effect and effect on Kupperman score and hormone levels in the treatment of low dose estrogen replacement therapy on the perimenopausal syndrome. 〔Methods〕132 patients were randomly divided into the study groups and the control groups, each group had 66 cases. The control groups were treated with oral administration of progesterone and medroxyprogesterone acetate, 4mg / d while the study groups oral estrogen Estradiol Valerate Tablets, 0. 65 mg / d on the basis of the control group. Two groups were treated continuously for 3 months. The Kupperman symptom score and the level of estradiol（ E2）, luteinizing hormone（ LH）, follicle stimulating hormone（ FSH） and uterine volume and endometrial thickness changes before and after treatment and adverse reactions between the two groupswere compared. 〔Results〕The Kupperman score in the research groups after 3 months treatment were significantly lower than that before treatment and the control groups（ P〈0. 05）.The level of E2 in the study groups were significantly higher than that before treatment and the control groups（ P〈0. 05） while the level of LH and FSH levels were significantly lower than those before treatment and the control groups（ P〈0. 05）. The uterine volume and endometrial thickness and adverse drug reaction among before and after treatment and the control groups had no significant difference（ P〉0. 05）. 〔Conclusion〕The low dose estrogen replacement therapy could significantly improve the clinical symptoms of perimenopausal syndrome, promote the normal secretion of hormones in the body and less adverse reaction, has remarkable curative effect.
