怀旧-认知行为干预对老年人抑郁症状和认知功能的影响

Effects of reminiscence-cognitive behavioral intervention on depressive symptom and cognitive function for older adults

目的探讨怀旧-认知行为干预对老年人抑郁症状和认知功能的影响。方法将91名具有轻中度抑郁症状的社区老年人随机分为对照组(31名)、怀旧组(30名)和怀旧联合认知行为组(30名)。对照组接受社区常规健康教育,怀旧组进行怀旧治疗,怀旧联合认知行为组同时进行怀旧治疗和认知行为干预。在干预前、干预4个月末使用老年抑郁量表(GDS)、简易精神状态量表(MMSE)、蒙特利尔认知评估量表北京版(MoCA)测量各组抑郁症状和认知功能。结果怀旧治疗组及怀旧联合认知行为干预组干预后GDS、MMSE及MoCA得分显著优于对照组,且怀旧联合认知行为干预组效果优于单纯怀旧治疗组(均P<0.05)。结论怀旧治疗及怀旧联合认知行为干预均能改善老年人的抑郁症状和认知功能,怀旧联合认知行为干预效果优于单纯怀旧治疗。

Objective To explore the effects of reminiscence-cognitive behavioral intervention on depressive symptom and cognitive function for older adults.Methods A total of 91community-dwelling older adults with mild or moderate depressive symptom were randomized into a control group（n=31）,a reminiscence group（n=30）and a reminiscence-cognitive behavioral group（n=30）.The control group received routine community health education,and the reminiscence group received reminiscence therapy,while the other group was given reminiscence therapy combined with cognitive behavioral intervention.Depressive symptom and cognitive function of the three groups were measured before and 4months after intervention using Geriatric Depression Scale（GDS）,MiniMental State Examination（MMSE）and Montreal Cognitive Assessment（MoCA）.Results The scores of GDS,MMSE and MoCA in the reminiscence therapy group and the reminiscence-cognitive behavioral group after the interventions were significantly better than the control group,and the effect of the reminiscence-cognitive behavioral intervention group was significantly better than the reminiscence therapy group（all P〈0.05）.Conclusion Reminiscence therapy and reminiscence-cognitive behavioral intervention could improve depressive symptom and cognitive function for older adults,and the effect of reminiscence-cognitive behavioral intervention is better.
