摘要 目的观察转录因子激活蛋白(AP-2α)、E-钙黏附素(E—cadherin)、内皮素-1(ET-1)、血管性假血友病性因子(vWF)在子痫前期孕妇血浆中的变化特点及意义。方法选取2014年4月—2015年9月广州市妇女儿童医疗中心妇产科诊治子痫前期患者71例为观察组,其中轻度亚组22例,早发重度亚组25例,晚发重度亚组24例,同时选取健康产妇22例为健康对照组,观察各组AP-2α、E-cadherin、ET-1、vWF表达水平并进行比较。结果健康对照组、轻度亚组、早发重度亚组、晚发重度亚组的AP-2α水平分别为(8.43±1.32)ng/ml、(14.53±2.24)ng/ml、(27.36±5.45)ng/ml、(19.37±3.19)ng/ml,E-cadherin水平分别为(90.35±6.35)ng/ml、(105.32±7.87)ng/ml、(135.57±8.97)ng/ml、(124.21±7.99)ng/ml,ET-1水平分别为(17.45±2.46)ng/ml、(25.69±3.15)ng/ml、(43.15±5.24)ng/ml、(37.03±4.42)ng/ml,vWF水平分别为(93.02±5.78)ng/ml、(107.32±7.43)ng/ml、(135.32±10.45)ng/ml、(125.46±8.97)ng/ml,与健康对照组比较,子痫前期各组上述4项指标水平均显著升高,晚发与早发重度亚组均高于轻度亚组,早发重度亚组均高于晚发重度亚组,差异均有统计学意义(P<0.05)。子痫前期孕妇血浆AP-2α、E-cadherin、ET-1、vWF等4种因子间两两采取Spearman进行相关性分析,AP-2c与其他3种指标呈正相关(r=0.797、0.775、0.722,P<0.05),E—cadheri与ET-1、vWF呈正相关(r=0.914、0.942,P<0.01),ET-1与vWF呈正相关(r=0.976,P<0.01)。结论AP-2α、E-cadherin、ET-1、vWF的高表达和子痫前期发病机制存在密切关联性,AP-2α能对E—cadherin、ET-1、vWF的表达产生影响,尤其是在子痫前期发病中具有重要的意义。 Objective To observe the application value of transcription factor activator protein(AP-2α),E-cadherin,endothelin-1(ET-1),von Willebrand factor(vWF) on the change characteristics of plasma in preeclampsia women and its significance.Methods 71 patients with preeclampsia from April 2014 to September 2015 in Guangzhou women and children medical center(22 cases of mild subgroup,25 cases of early onset severe subgroup,24 cases of late onset severe subgroup)were enrolled,meanwhile,22 cases of healthy women were selected as control group,AP-2α,E-cadherin,ET-1,vWF expression levels of each group was observed and compared.Results AP-2α levels of the control group,mild subgroup,the early-onset of severe subgroup and late-onset severe subgroup were respectively(8.43 ± 1.32) ng/ml,(14.53 ± 2.24)ng/ml,(27.36±5.45) ng/ml,(19.37±3.19) ng/ml,E-cadherin levels respectively(90.35±6.35) ng/ml,(105.32±7.87) ng/ml,(135.57 ± 8.97) ng/ml,(124.21 ±7.99) ng/ml,E-cadherin level respectively(17.45 ±2.46) ng/ml,(25.69 ± 3.15) ng/ml,(43.15 ± 5.24) ng/ml,(37.03 ± 4.42) ng/ml,vWF levels respectively(93.02 ± 5.78) ng/ml,(107.32 ± 7.43) ng/ml,(135.32 ± 10.45) ng/ml,(125.46 ± 8.97) ng/ml.Compared with the healthy control group,the levels of the above 4 indexes in preeclampsia group were significantly higher than those in the control group.Late-onset and early-onset severe group were higher than mild group,early-onset severe subgroup were higher than that of late-onset severe group,there were statistically significant differences(P〈0.05).Plasma AP-2α,E-cadherin,ET-1,vWF expression in preeclampsia women were analysed by Spearman.There was a positive correlation between AP-2α and other 3 indexes(r =0.797,r =0.775,r =0.722,P〈0.05).E-cadheri has a positive correlation with ET-1 and vWF(r =0.914,r =0.942,P〈0.01),and ET-1 has a positive correlation with vWF(r =0.976,P〈0.01).Conclusion The high expressions of AP-2α,E-cadherin,ET-1,and vWF have a close correlation between the pathogenesis of preeclampsia,AP2αcan influence the expressions of ET-1 and vWF,especially plays an important role in the incidence of preeclampsia.
