声脉冲辐射力成像对乳腺良恶性结节的鉴别诊断价值 被引量:8

摘要 目的 :探讨声脉冲辐射力成像(acoustic radiation force impulse,ARFI)在诊断乳腺良恶性结节中的价值。方法:分析128例共145个经病理证实的乳腺结节的声脉冲辐射力成像特征。采用ROC曲线评价剪切波速度值对乳腺病灶良恶性的鉴别诊断价值,并确定临界值。结果:145个结节中,良性结节107个,恶性结节38个。乳腺病灶良性组与恶性组的剪切波速度(shearwave velocity,SWV)值分别为(2.89±1.13)m/s和(6.57±1.36)m/s,2组差异有统计学意义(P<0.05)。以SWV值3.37 m/s为临界值诊断乳腺病灶的良恶性的敏感度、特异度、准确度分别为89.4%、89.7%、90.3%。结论:ARFI为乳腺结节的良恶性鉴别诊断提供了新的分析方法,具有重要的临床价值。 Objective:To explore the value of acoustic radiation force impulse (ARFI) imaging in the diagnosis of benign and malignant breast lesion. Methods:Analysed the features of ARFI of 128 cases with 145 breast lesions which confirmed by pathology. The cut-off values were determined by the analysis of receiver-operating characteristic (ROC) curve using pathology as the golden criteria. Results:There were 107 benign lesions and 38 malignant lesions in 145 lesions.The shear-wave velocity (SWV) value of benign and malignant breast lesions were (2.89 ± 1.13)m/s and(6.57 ± 1.36)m/s. There was significant difference in the SWV value between benign and malignant breast lesions (P〈0.05). When the diagnostic threshold was 3.37m/s,the sensitivity,specificity and accuracy of SWV values between benign and malignant breast lesion was 89.4%,89.7%,90.3%,respectively. Conelusion:ARFI provides a new analysis method for the differential diagnosis of benign and malignant breast lesions,and has important clinical value.
