抗早颗粒对性早熟女童初潮年龄和成年终身高的影响

Effects of Kangzao Granule on Age of Menarche and Final Adult Height in Girls with Precocious Puberty

目的通过长期随访,比较化痰泻火中药抗早颗粒和西药达菲林对特发性中枢性性早熟(idiopathic central precocious puberty,ICPP)女童初潮年龄及终身高的影响。方法回顾性分析60例ICPP女童的临床及随访资料,中药组和西药组各30例,均已达到或接近成年终身高(final adult height,FAH)。比较两组患者治疗前后身高、预测成年身高(predictive adult height,PAH)、FAH、靶身高(target height,TH)、剩余生长能力、身高净增值、初潮年龄以及月经间期的差异。结果与治疗前比较,两组患者治疗后身高和PAH均显著增加(P<0.05),中药组患者身高增加值显著大于西药组(P<0.05);两组患者FAH、TH、剩余生长能力、身高增净值、初潮年龄和月经间期比较,差异均无统计学意义(P>0.05)。在相同疗程下,中药组终身高略低于西药组,但两组间差异无统计学意义(P>0.05)。结论在改善患儿终身高和控制初潮年龄方面,中药和西药的效果相似,但中药较西药价廉而且不良反应少,因而比西药更有优势。

Objective To compare the effects of phlegm-resolving and fire-purging traditional Chinese medicine （TCM） Kangzao Granule and the Western medicine （WM） Diphereline on the age of menarche and final adult height in girls with idiopathic central precocious puberty （ICPP） through a long-term follow-up. Methods A retrospective analysis was performed on the clinical and follow-up data of 60 girls with ICPP,who were equally divided into TCM group and WM group. All of the cases achieved or almost achieved their final adult height （FAH）. Height and predictive adult height （PAH） before and after therapy, FAH, target height （TH）, remaining growth ability, net height increase, the age of menarche, and inter-menstrual period were compared between the two groups. Results Both groups showed significant increases in height and PAH after treatment （P〈0.05）, and the TCM group had a significantly greater increase in height than the WM group （P〈0.05）. FAH, TH, remaining growth ability, net height increase, the age of menarche, and intermenstrual period showed no significant differences between the two groups （P〉0.05）. With the same course of treatment, the TCM group had a slightly lower FAH than the WM group （P〉0.05）. Conclusion For girls with ICPP, Kangzao Granule has similar efficacy as Diphereline in improving FAH and controlling the age of menarche. However, Kangzao Granule has a lower price and causes fewer adverse reactions compared with Diphereline. Therefore, Kangzao Granule is superior to Diphe- reline in the treatment of ICPP.
