耳穴贴压联合中药灌肠治疗慢性非特异性溃疡性结肠炎临床疗效及对患者生活质量的影响

Study of the effects of Auricular application combined with Chinese medicine enema on the treatment of chronic nonspecific ulcerative colitis and its effects on patients quality of life

目的观察耳穴贴压联合中药灌肠治疗慢性非特异性溃疡性结肠炎(CUC)的临床疗效及对患者生活质量的影响。方法将125例CUC患者按随机数字表法分为3组。对照组40例,予美沙拉秦肠溶片治疗;灌肠组41例,在对照组的基础上加用改良愈疡汤灌肠治疗;耳穴贴压联合灌肠组(以下简称联合组)44例,在灌肠组基础上加用耳穴贴压治疗。3组均治疗2周后比较疗效,并采用中文版炎症性肠病问卷(IBDQ)评分(包括肠道症状、全身症状、情感功能及社会功能4个方面)对患者治疗前后的生活质量进行比较。结果联合组总有效率97.72%,灌肠组总有效率82.92%,对照组总有效率62.50%,3组总有效率比较差异有统计学意义(P<0.05),联合组最高,其次是灌肠组、对照组;3组治疗后IBDQ总分与本组治疗前比较均有明显增加(P<0.05),联合组增加最多,其次是灌肠组、对照组,且联合组治疗后对情感功能的改善也明显高于灌肠组和对照组(P<0.05)。结论耳穴贴压联合中药灌肠治疗CUC可明显提高临床疗效,提高IBDQ评分,改善患者生活质量。

Objective To observe the clinical effects of Auricular application combined with Chinese medicine enema on the treatment of chronic nonspecific ulcerative colitis,and its effects on patients quality of life.Methods 125 subjects were divided into three groups in accordance with the random number table.40 subjects in control group were treated by Mesalazine enter-coated tablets.41 subjects in enema group were treated by modified Yuyang decoction enema on the basis of control group treatment.44 subjects in Auricular application combined with Chinese medicine enema group（ combination group） were treated by Auricular application on the basis of enema group treatment.The curative effect was evaluated two weeks after treatment in three groups.The living quality before and after treatment were evaluated by the inflammatory bowel disease questionnaire（ IBDQ） in Chinese version,including the symptoms of bowel and whole body,emotional and social function.Results The total effective rate in combined,enema and control groups were 97.72%,82.92% and 62.50% respectively,with statistical significance（ P〈0.05）,combined group was highest,followed by enema and control group.The total scores of IBDQ after treatment were obviously increased in three groups（ P〈0.05）,the increase in combined group was highest,followed by enema and control groups.The improvement of emotional function after treatment in combined group was obviously better than that in enema and control groups（ P〈0.05）.Conclusion Auricular application combined with Chinese medicine enema can significantly enhance the clinical effects on chronic nonspecific ulcerative colitis,increase IBDQ scores,and improve the quality of life.
