脊柱手术围手术期抗菌药物预防应用干预效果评价

An Evaluation of Intervention Effect to Antibiotic Prophylaxis During Perioperative Period of Spinal Surgery

目的探讨围手术期抗菌药物预防用药合理性及抗菌药物临床应用专项整治活动的干预效果。方法选取北京市某三级医院脊柱I类切口手术病例整治前2011年1—3月673例及整治后2014年1—3月974例进行对比分析,比较抗菌药物预防应用情况。结果整治后,抗菌药物的首次用药时机、用药疗程、品种选择、给药途径、联合用药的合理率分别由整治前的6.61%、2.67%、7.73%、10.25%、8.62%提升至50.93%、49.79%、91.58%、94.35%、91.38%,差异均有统计学意义(P<0.05)。结论整治活动对于规范脊柱I类切口手术围手术期抗菌药物预防应用很有成效,明显提高用药合理性。

Objective To explore the changes of using antimicrobial drugs during surgical perioperative period after the promulgation of The Modified Clinical Application of Antibacterial Drugs Action Plan and analyze the intervention effect.Methods We identified 974 medical records of patient who received spinal surgery whereas type I incision from January to march,2014 and compared it with 974 medical records of patient who received spinal surgery whereas type I incision from January to march,2011 from the same tertiary hospital in Beijing. Results After the promulgation of The Modified Clinical Application of Antibacterial Drugs Action Plan,the timing accuracy increased from 6.61% to 50.93%.The duration become shorter. The selection accuracy increased from 7.7% to 91.4%. The route of administration accuracy increased from 10.3% to 94.2%. The combination accuracy increased from 8.6% to 90.8%. Conclusion After the promulgation of The Modified Clinical Application of Antibacterial Drugs Action Plan,practices of antimicrobial drugs use in spinal surgery whereas type I incision during surgical perioperative period has a obviously positive change.
