结核性包裹性胸腔积液的治疗与预防心得探讨

Treatment and prevention of tuberculous encapsulated pleural effusion

目的 探讨微创中心静脉导管胸腔内置入引流治疗结核性包裹性胸腔积液,浆膜腔应用尿激酶的方法及临床效果.常规治疗组采用反复胸腔穿刺抽液方法,引流注药组在常规治疗基础上行微创中心静脉导管胸腔内置引流尿激酶胸腔内注射,进行综合比较.结果显示引流注药组胸水引流量显著增加,胸膜肥厚、临床症状、并发症、住院时间的改善,均优于常规治疗组.微创中心静脉导管胸腔内置引流行尿激酶胸腔内注易实施且射安全有效创伤小,可作为包裹性结核性胸膜炎的治疗首选方案.

Objective insert drainage to central venous catheter chest in treatment of tuberculous pleural effusion, and the methods and clinicaleffect of serosal application of urokinase. MethodThe routine treatment group used repeated pleural puncture pumping method,drainage group on thebasis of routine treatment, for that minimally invasive central venous catheter indwelling drainage urokinase intrapleural injection,than comprehensivecomparison.Results the drainage and injecting drugs significantly increased pleural effusion drainage, and thickening of the pleura, symptoms disappearedquickly, fewer complications, its effect is better than that of routine treatment group.Conclusion： minimally invasive central venous catheter placed intothe drainage and intrapleural urokinase injection is a safe and effective, less trauma, easy to implement,and is the preferred treatment of encapsulatedtuberculous pleurisy.
