抗生素管理及手卫生对新生儿血液细菌感染的影响 被引量:1

摘要 目的对新生儿血液细菌感染进行抗生素以及手卫生的规范管理,并探讨其临床效果。方法抽取2012年3月—2015年3月该院接诊的血液细菌感染的80例新生儿作为研究对象,在此期间对医院病房进行抗生素以及手卫生的整治管理,分别观察整治前后医护人员手卫生的依从情况,新生儿的抗生素使用强度,并比较治疗前后新生儿血液CRP及PCT值进行检测与对比。结果整治前抗生素的使用强度(11.8±2.1)明显高于整治后的(7.9±1.0),且碳青霉烯类使用强度(0.9±0.2)也显著高于整治后的(0.7±0.1),组间比较差异具有统计学意义(t=10.605,5.657,P<0.05);整治后医护人员的手卫生依从度在医师、护士以及接触患儿前三个方面均显著高于整治前,组间比较差异具有统计学意义(χ2=5.541,10.313,4.943,P<0.05);本研究中,新生儿整治前其CRP及PCT检测值显著高于整治后,比较差异具有统计学意义(t=16.144,21.168,P<0.05)。结论对新生儿病房采取抗生素管理以及手卫生规范措施后可有效降低抗生素的使用强度,降低其血液CRP及PCT检测值,从而有效降低医院交叉感染的发生率,提高新生儿质量,减少医患纠纷。 Objective To neonatal blood bacterial infections to antibiotics and hand hygiene standard management, and to explore its clinical effect. Methods The extraction in March 2012- March 2015 in our hospital departments of 80 cases of neonatal blood bacterial infections as the research object, in the meantime to antibiotics and hospital wards improvement of hand hygiene management, before and after the renovation of medical staff hand hygiene compliance were observed, the newborn intensity of use of antibiotics, and compare the before and after treatment of neonatal blood CRP and PCT values for testing and comparison. Results The intensity of the use of antibiotics before renovation(11.8 ±2.1) was significantly higher(7.9±1.0), after the renovation of penicillium and carbon alkene using intensity(0.9±0.2) was also significantly higher than(0.7±0.1), after the renovation of more significant difference between groups, statistically significant(t=10.605, 5.657,P〈0.05); After regulation of medical staff hand hygiene adherence in children with doctors, nurses and contact the former three aspects were significantly higher than that of before the renovation, more significant difference between groups, statistically significant(χ^2=5.541, 10.313,5.541,P〈0.05); In this study, the newborn regulation before the CRP and PCT values were significantly higher after regulation, more significant statistical significance difference(t=16.144,21.168,P〈0.05). Conclusion Neonatal wards after antibiotic management and hand hygiene standard measures can effectively reduce the intensity of the use of antibiotics, reduce the blood CRP and PCT values, so as to reduce the incidence rate of hospital cross infection effectively, improve the quality of the newborn, reduce medical disputes.
