膝关节镜治疗创伤性膝关节滑膜炎手术患者的护理管理

The Arthroscopic Treatment of Patients with Traumatic Synovitis of Knee joint Surgery Nursing Management

目的观察膝关节镜治疗创伤性膝关节滑膜炎手术患者的护理管理。方法将该院2014年2月—2015年9月收治的72例创伤性膝关节滑膜炎患者随机分为两组,对照组和观察组各36例,均采用膝关节镜手术治疗。对照组给予常规护理,观察组在常规护理的基础上给予综合护理。分别于护理前和护理后,检测患者的膝关节周径、膝关节屈伸度,评估疼痛程度,并检测患者膝关节液中炎症因子,分析两组的临床护理疗效。结果观察组的总有效率为97.22%,明显高于对照组的66.67%,差异无统计学意义(P<0.05);两组患者护理前的膝关节周径、膝关节屈伸度和VAS评分对比,差异无统计学意义(P>0.05);观察组患者护理后的膝关节周径、膝关节屈伸度和VAS评分分别为(33.08±2.25)cm、(88.69±17.64)o、(2.97±0.82)分,明显优于对照组的(37.38±3.41)cm、(77.66±16.95)°、(5.83±1.31)分,差异具有统计学意义(P<0.05);两组患者护理前的IL-1、IL-6和TNF-α对比,差异无统计学意义(P>0.05);观察组患者护理后的IL-1、IL-6和TNF-α分别为(18.47±6.82)pg/m L、(42.18±7.50)pg/m L、(8.29±2.54)pg/m L,明显优于对照组的(32.46±6.70)pg/m L、(53.28±7.83)pg/m L、(12.38±2.91)pg/m L,差异具有统计学意义(P<0.05)。结论给予膝关节镜治疗创伤性膝关节滑膜炎手术患者科学的综合护理,疗效确切,有利于患者膝关节功能恢复,值得推广应用。

Objective to observe the arthroscopic treatment of patients with traumatic synovitis of knee joint surgery nursing management. Methods from February 2014 to september 2015, 72 patients with traumatic synovitis of knee joint were randomly divided into two groups, control group and observation group 36 cases, adopt the arthroscopic surgery. Control group given conventional nursing, the observation group on the basis of conventional nursing care to comprehensive care. In nursing before and after, detection in patients with knee joint weeks diameter, degree of knee flexion and extension, evaluate the degree of pain, and detection of inflammatory factors in patients with knee joint fluid, analysis of the clinical nursing efficacy of two groups. Results observation group total effective rate was 97.22%, significantly higher than the control group66.67%, significant difference（P〈0.05）; Two groups of patients with knee weeks before treatment diameter, degree of knee flexion and extension, and VAS score comparison, no significant difference（P〉0.05）; Observation group of patients after nursing knee weeks diameter, degree of knee flexion and extension, and VAS score（33.08 ＋ 33.08） cm, respectively（88.69＋ 17.64） DHS,（2.97-0.82）, significantly better than that of control group（37.38 ＋ 37.38） cm, DHS（77.66 ＋ 16.95）,（5.83-1.31）, significant difference（P〈0.05）; Two groups of patients before treatment of IL- 1, IL 6 and TNF alpha contrast, no significant difference（P〉0.05）; Observation group after nursing care to patients, IL- 6 and IL- 1 TNF alpha（18.47 ＋6.82）, respectively, pg/m L, pg/m L（42.18 ＋ 7.50）,（8.29-2.54） pg/m L, significantly better than that of control group（32.46 ＋6.70） pg/m L, pg/m L（53.28 ＋ 7.83）,（12.38-2.91） pg/m L, significant difference（P〈0.05）. Conclusion for the arthroscopic treatment of patients with traumatic synovitis of knee joint surgery scientific comprehensive nursing, curative effect, is beneficial to patients with knee joint functional recovery, is worthy of popularization and application.
