分析无创与有创正压通气在COPD伴Ⅱ型呼吸衰竭治疗中的应用

Application Analysis of Noninvasive and Invasive Positive Pressure Ventilation in Treatment of COPD with Type II Respiratory Failure

目的分析COPD伴Ⅱ型呼吸衰竭治疗中应用无创与有创正压通气的效果。方法从该院2014年9月—2015年9月期间收治的COPD伴Ⅱ型呼吸衰竭患者中随机抽取62例分为两组,每组均31例,对照组单纯采用有创正压通气治疗,实验组则采用用有创无创续贯正压通气治疗,分析两组临床应用价值。结果治疗24 h后,实验组Pa O2和p H值上升程度、Pa CO2和HR下降程度、通气时间以及住院天数等均优于对照组,差异有统计学意义(P<0.05)。结论有创无创续贯正压通气治疗治疗COPD伴Ⅱ型呼吸衰竭的临床效果较好,更有利于患者的病情的恢复。

Objective To analyze the effect of the application of noninvasive and invasive positive pressure ventilation in treatment of COPD with type II respiratory failure. Methods 62 cases of patients with COPD with type II respiratory failure treated in our hospital from September 2014 to September 2015 were extracted and divided into two groups with 31 cases in each, the control group were treated with simple invasive positive pressure ventilation, the experimental group were treated with noninvasive and invasive continued positive pressure ventilation, the clinical application value of the two groups was analyzed. Results After 24 h of treatment, the Pa O2, p H value increase degree, Pa CO2, HR decrease degree, duration of ventilation and length of stay in the experimental group were better than those in the control group. and the comparison was statistically significant（P〈0.05）. Conclusion The noninvasive and invasive continued positive pressure ventilation in treatment of COPD with type II respiratory failure has a better clinical effect, and it is more conducive to the recovery of the patient＇s condition.
