摘要
目的系统评价自动乳腺全容积扫查(ABVS)系统对乳腺癌的诊断价值。方法检索万方、CNKI、Pub Med、Web of science及EMbase数据库中2014年12月前公开发表的有关ABVS对乳腺病灶诊断的文献,按诊断性试验的纳入和排除标准筛选文献、提取数据,进行Meta合并分析。结果共计19篇文献2073个乳腺病灶纳入分析,采用随机效应模型合并敏感性、特异性、阳性似然比、阴性似然比及诊断比值比分别为0.87[95%CI(0.85~0.89)]、0.87[95%CI(0.85~0.89)]、5.98[95%CI(4.72~7.57)]、0.16[95%CI(0.11~0.23)]、51.81[95%CI(33.32~80.57)];SROC曲线下面积为0.94±0.01,Q*指数为0.88±0.01。结论 ABVS对乳腺癌有较高敏感性和特异性,对乳腺病灶的良、恶性鉴别诊断有较高的临床诊断价值。
Objective To assess the clinical diagnostic value by automated breast volume scanner(ABVS)in breast cancer by meta-analysis method.Methods Literatures were searched on electronic database of Wan Fang,CNKI,Pub Med,and Web of science and EMbase published before December 2014. Each article was reviewed to identify according to the diagnostic criteria,and data were analyzed by meta-analysis.Results Totally 2073 lesions in 19 articles were enrolled.The pooled sensitivity, specificity,positive likelihood ratio and negative likelihood ratio were 0.87[95%CI(0.85~0.89)],0.87[95%CI(0.85~0.89)],5.98[95% CI(4.72 ~7.57)],0.16[95% CI(0.11 ~0.23)],51.81[95% CI(33.32 ~80.57)],respectively. The area under SROC curve was 0.94 ±0.01,and the Q-index was 0.88 ±0.01.Conclusion ABVS system has high sensitivity and specificity in breast cancer,and has diagnostic value in differential diagnosis of breast diseases.
