摘要 目的探讨自控输注泵用于晚期癌痛患者镇痛的临床观察。方法实验组对26例晚期癌痛患者应用自控输注泵经皮下注射治疗。给药配方:吗啡40 mg+2%利多卡因20 ml+氟哌利多卡因2.5 mg+0.9%氯化钠至100 ml镇痛,输注速度为2 ml/h,自控0.5 ml/次,锁定时间为15 min,对照组26例采用传统方法给于肌肉注射吗啡0.1~0.2mg/kg,并根据血中药物浓度及半衰期重复使用,采用视觉模拟评分法(VAS)评价镇痛效果。结果实验组26例中显效21例占80.76%,有效3例占11.53%,无效2例占7.69%,总有效率为92.29%。对照组26例中显效13例占50%,有效10例占38.46%,无效3例占11.53%,总有效率为88.46%。两组比较差异有统计学意义,P〈0.05。结论自控输注泵用于晚期癌痛患者镇痛方法简便、易行且安全,镇痛效果好。 Objective To explore the clinical observation methods of analgesia for patients using autocontrol infusion pump with advanced cancer: the experimental group was 26 cases of advanced cancer patients who were subcutaneous injected by using autocontrol infusion pump. Methods In the experimental group, 26 patients were treated by subcutaneous injection. 40 ml Morphine+2% 20 ml lidocaine+2.5 mg lidocaine droperidol+0.9% 100 ml sodium chloride for analgesia, the rate of infusion is 2 ml/h and autocontrol is 0.5 ml/times. Time was set to 15 minutes. Relatively, the control group consisting of 26 patients with advanced cancer were taken a traditional treatment by intramuscular injection of 0.1~0.2 mg/kg morphine and reused it according to the concentration of drugs in the blood and drugs' half-period and evaluated the effectiveness of analgesia by applying visual analogue scale(VAS). Results In experimental group, 21 cases were markedly effective 80.76%, 3 cases were effective 11.53%, 2 cases were ineffective 7.69% and total effective rate is 92.29%. In control group, 13 cases were markedly effective 50%, 10 cases were effective 38.46%, 3 case were ineffective 11.53% and total effective rate id 88.46%. It is statistically significant to compare the differences in markedly effectiveness between two groups. Conclusion By using autocontrol infusion pump for patients with advanced cancer to alleviate pain is simple, easy, safe and effective.
