他汀类药物治疗脑出血合并高脂血症患者初步探讨

Investigation on statin therapy in patients with hyperlipidemia combined with cerebral hemor-rhage

目的：探讨他汀类药物治疗脑出血合并高脂血症患者的临床疗效及安全性。方法回顾性分析2012年7月至2014年7月在漯河市郾城区人民医院治疗的78例脑出血患者的临床资料，随机双盲法分为研究组和对照组，研究组采用他汀类药物治疗，对照组给予常规内科治疗，比较两组治疗前后血脂指标[总胆固醇（ TC）、三酰甘油（TG）、低密度脂蛋白胆固醇（LDL-C）等]及丙氨酸氨基转移酶（ALT）、肌酸激酶（CK）等水平变化。结果研究组TC、TG、LDL-C等水平均较治疗前下降，差异有统计学意义（P〈0．05）；对照组TC、TG、LDL-C等水平均较治疗前略有下降，但差异未见统计学意义（P〉0．05）。两组治疗前后ALT、CK等水平波动不明显，差异未见统计学意义（P〉0．05）。两组治疗过程中均出现1例再次脑出血，无心肌梗死和死亡病例。结论他汀类药物对改善脑出血合并高脂血症患者血脂水平、预防脑卒中和脑复发疗效确切，对肝肾功能损伤小，不良反应少，是治疗脑出血合并高脂血症理想的药物。

Objective To investigate the clinical efficacy and safety in patients with iscussion statin treatment of cerebral hemorrhage complicated with hyperlipidemia. Methods Retrospective ana-lyzed the clinical data of 78 patients with cerebral hemorrhage from July 2012 to July 2014 , and divided them into study group and control group randomizedly double-blind. The study group received statin ther-apy, and the control group received conventional medical treatment. Serum lipids （ TC, TG, LDL-C, etc. ） and proglumetacin aminotransferase, and creatine kinase levels before and after treatment of two groups were compared. Results TC, TG, LDL-C and other levels in study group decreased than before treatment, the difference was significant（P〈0. 05）. TC, TG, LDL-C and other levels declined slightly compared with before treatment in control group, but there was no significant difference （ P 〉0. 05 ） . ALT, CK levels before and after treatment had no significant differences （P〉0. 05）. One case of cere-bral hemorrhage both occurred in the two groups over treatment with no myocardial infarction and death. Conclusions Statins can effectively improve blood lipid levels in patients with cerebral hemorrhage hyper-lipidemia, preven the recurrence of stroke patients and the precise brain, liver and kidney function damage with fewer adverse reactions, and is ideal for the treatment of cerebral hemorrhage hyperlipidemia drugs.
