现代康复联合JAS对肘关节“恐怖三联征”患者术后康复的影响

Effect of Modern Rehabilitation Combined with JAS on Postoperative Rehabilitation of Patients with “Terrible Triad” of Elbow Joint

目的观察现代康复联合JAS对肘关节＂恐怖三联征＂患者术后康复的影响。方法对我院骨外科-康复科2012年至2015年收治的肘关节＂恐怖三联征＂患者40例,采用随机化数字法分为观察组20例和对照组20例。两组患者均采用手术治疗,术后固定4~6周,外固定撤除后进行肘关节常规物理因子、康复功能训练、关节松动等综合康复治疗。观察组在此基础上运用JAS肘关节屈伸训练器进行静态渐进牵伸治疗。两组患者康复治疗12周后分别进行Mayo肘关节功能评分和肘关节活动度（ROM）评定。结果两组患者康复治疗12周后进行Mayo肘关节功能评分及肘关节活动度（ROM）评定显示：观察组Mayo肘关节功能评分、ROM评定均优于对照组,差异有统计学意义（P〈0.05）;两组随访1年,肘关节活动障碍例数差异有统计学意义（P〈0.05）。结论现代康复联合JAS肘关节屈伸训练器对肘关节＂恐怖三联征＂功能障碍患肘进行静态渐进牵伸有显著促进作用,能较好地恢复肘关节功能。

Objective To observe the effect of modern rehabilitation combined with JAS on postoperative rehabilitation of patients with＂terrible triad＂of elbow joint. Methods 40 cases of patients with ＂ terrible triad＂ of elbow joint admitted to the department of bone surgery and rehabilitation in our hospital from 2012 to 2015 were divided into observation group（ 20 cases） and control group（ 20 cases） with random number method. Two group were treated with operation and postoperative 4-6 weeks of fixation,and comprehensive rehabilitation treatment of elbow joint conventional physical factors,rehabilitation function training,joint mobilization and so on were carried out after the removal of external fixation. On this basis,observation group were treated with static progressive drawing treatment by JAS elbow flexion and extension training device. Mayo elbow function score and elbow joint mobility（ ROM） were evaluated in the two groups after 12 weeks of rehabilitation treatment. Results Mayo elbow function score and ROM of two groups after 12 weeks of rehabilitation treatment indicated that both mayo elbow function score（ P〈0. 01） and ROM（ P〈0. 05） in observation group were better than control group. The number of elbow joint movement disorders of two groups were different during 1 year follow-up（ P〈0. 05）. Conclusion Modern rehabilitation combined with JAS elbow flexion extension training device had significant promoting effect for patients with elbow joint ＂ terrorist triple sign＂ to conduct static progressive stretching,which could recover elbow function better.
