钛制弹性髓内钉治疗儿童四肢骨折所致并发症及预防对策

Application of titanium elastic nail for the treatment of children limbs fracture caused by complications and prevention countermeasures

目的探讨钛制弹性髓内钉(TEN)治疗儿童四肢骨折所致的并发症情况,并针对并发症制定预防对策。方法回顾性分析自2013年2月至2015年1月河北省遵化市东旧寨医院收治的88例四肢骨折患儿的临床资料,根据入院时间分为对照组44例,观察组44例。对照组患儿实施切开复位钢板内固定治疗,观察组患儿行TEN治疗。比较两组患儿手术情况,治疗后骨折愈合效果,并发症的发生等情况。结果观察组,术中出血量较低,切口长度短,住院时间及手术时间明显较短,术后并发症发生率(27.27%)明显低于对照组(52.27%),两组比较,差异有统计学意义(P<0.05)。结论 TEN治疗儿童四肢骨折,创伤小,并发症发生率低,术后恢复好。但仍然存在一系列并发症,给予针对性措施进行并发症预防,对提高手术效果,减轻患者痛苦,改善预后具有积极作用,值得推广。

Objective To explore the titanium elastic nails（ TEN） treatment of the complications of extremities fractures in children,and to develop prevention countermeasures against complications. Methods From February 2013 to January 2015,88 cases of limb fractures were retrospectively analyzed,according to admission time were divided into 44 cases of control group,44 cases of observation group. Control group with steel plate internal fixation（ orif）,observation group with TEN treatment. Two groups of children were compared in operation situation,fracture healing after treatment effect,complications,et. Results In the observation group,the bleeding was less,incision length was shorter,length of hospital stay and the operation time were significantly shorter,compared with the control group,the incidence of postoperative complications was 27. 27%,the observation group was obviously lower than the control group52. 27%,which was statistically significant（ P 〈 0. 05）. Conclusion The trauma of TEN limbs fracture treatment to children is small,relative complications are less,postoperative recovery is better; however,there are still a series of complications,which needs specific measures to prevent complications,to improve the effect of the operation,to reduce patient pain,to improve prognosis and to get a positive effect,which is worth being promoted.
