血清心型脂肪酸结合蛋白水平与慢性心力衰竭的相关性研究 被引量:4

摘要 目的:探讨血清心型脂肪酸结合蛋白(H-FABP)水平与慢性心力衰竭(CHF)的相关性。方法:选取CHF患者(CHF组,依纽约心脏病协会心功能分级标准为Ⅱ-Ⅳ级)及同期住院的非CHF患者(对照组)各60例,均抽血检测常规生化项目以及血清N末端B型利钠肽原(NT-pro BNP)和H-FABP水平,心脏彩色多普勒超声测定左室舒张末内径(LVEDD)及左室射血分数(LVEF),并详细记录入选者一般资料,采用统计学方法比较两组间差异。结果:CHF组血清H-FABP水平显著高于对照组(P〈0.05),且与NT-pro BNP水平及心功能级别呈正相关(r=0.72,r=0.56,P〈0.05);血清H-FABP水平与LVEDD呈正相关(r=0.69,P〈0.05),与LVEF呈负相关(r=-0.42,P〈0.05)。结论:血清H-FABP水平与CHF具有一定相关性,参与了CHF心室重构的发生发展,可以为CHF早期诊断、病情程度判断及预后评估提供一定的临床参考价值。 Objective: To investigate the correlation between serum level of heart- type fatty acid binding protein( H-FABP) and chronic heart failure( CHF). Methods: A total of 60 patients with chronic heart failure( CHF group,classed by NYHA Ⅱ - Ⅳ) and other 60 cases without heart failure( control group) were enrolled in the same period. Blood routine biochemistry project,serum level of NT- pro BNP and H- FABP was detected in both groups. The left ventricular end- diastolic diameter( LVEDD) and left ventricular ejection fraction( LVEF) were measured in all cases by cardiac color doppler ultrasonic and the baseline data were recorded in details. The data were compared by statistical analysis in both groups. Results: Significant increased of serum H- FABP level was noted in CHF group compared with control group( P〈0. 05),and indicated that it was positively correlated with the NT- pro BNP level and cardiac function classes( r = 0. 72,r = 0. 56,P〈0. 05). Serum level of H- FABP was positively correlated with LVEDD( r = 0. 69,P〈0. 05),but negatively correlated with LVEF( r =- 0. 42,P〈0. 05). Conclusion: The certain relevance may be found between serum level of H- FABP and CHF in this investigation. H- FABP may participate in the occurrence and ventricular remodeling of CHF,which can provide certain clinical reference value for early diagnosis,a degree of judgment and prognosis assessment of CHF.
